摘　　要：目的:评估酒石酸美托洛尔联合厄贝沙坦氢氯噻嗪治疗老年重症心力衰竭的效果。方法:选取2023年1—12月于临沂市中心医院接受治疗的老年重症心力衰竭患者90例为研究对象,随机分为参考组和研究组,每组45例。两组均接受常规抗心力衰竭综合治疗,在此基础上,参考组使用酒石酸美托洛尔治疗,研究组在参考组基础上使用厄贝沙坦氢氯噻嗪治疗。比较两组心功能[心排血量(CO)、心排血指数(CI)、左心室射血分数(LVEF)和脑钠肽(BNP)水平]、治疗效果及不良反应发生率。结果:治疗前,两组CO、CI、BNP、LVEF水平比较,差异无统计学意义(P>0.05);治疗后,两组CO、CI、LVEF水平升高,BNP水平降低,研究组优于参考组(P<0.05)。研究组治疗总有效率高于参考组(P=0.030)。研究组不良反应发生率低于参考组(P=0.019)。结论:酒石酸美托洛尔联合厄贝沙坦氢氯噻嗪治疗老年重症心力衰竭的效果较好,能够提高患者心功能,减少不良反应。Objective:To evaluate the efficacy of metoprolol tartrate combined with irbesartan hydrochlorothiazide in the treatment of severe heart failure in elderly patients.Methods:Ninety elderly patients with severe heart failure treated at Linyi Central Hospital from January to December 2023 were selected and randomly divided into a reference group and a study group,with 45 cases in each group.Both groups received conventional comprehensive anti-heart failure therapy.The reference group was additionally treated with metoprolol tartrate,while the study group received irbesartan hydrochlorothiazide on the basis of the reference group's treatment.Cardiac function[cardiac output(CO),cardiac index(CI),left ventricular ejection fraction(LVEF),and brain natriuretic peptide(BNP)levels],therapeutic efficacy,and incidence of adverse reactions were compared between the two groups.Results:Before treatment,there were no statistically significant differences in CO,CI,BNP,or LVEF levels between the two groups(P>0.05).After treatment,CO,CI,and LVEF levels increased,and BNP levels decreased in both groups,with the study group showing superior outcomes compared to the reference group(P<0.05).The total effective rate of the study group was higher than that of the reference group(P=0.030).The incidence of adverse reactions in the study group was lower than that in the reference group(P=0.019).Conclusion:Metoprolol tartrate combined with irbesartan hydrochlorothiazide demonstrates favorable efficacy in treating severe heart failure in elderly patients,improving cardiac function and reducing adverse reactions.
