摘 要:目的:比较抗蝮蛇毒血清与抗五步蛇毒血清治疗竹叶青蛇咬伤的效果。方法:选取黔西南布依族苗族自治州人民医院2023年1月—2025年1月收治的74例竹叶青蛇咬伤患者为研究对象,以随机数字表法分为对照组、观察组,各37例。两组均接受常规治疗,在此基础上,对照组采用抗蝮蛇毒血清治疗,观察组采用抗五步蛇毒血清治疗。比较两组凝血功能、炎性指标水平以及并发症发生情况。结果:治疗后3 d,两组凝血酶原时间、活化部分凝血活酶时间缩短,纤维蛋白原水平升高,D-二聚体水平降低,观察组优于对照组(P<0.05);治疗后3 d,两组白细胞计数、C反应蛋白水平降低,观察组低于对照组(P<0.05);观察组并发症总发生率低于对照组(P<0.001)。结论:抗五步蛇毒血清治疗竹叶青蛇咬伤的效果优于抗蝮蛇毒血清,可改善患者的凝血功能,减轻炎性反应,并降低并发症发生率。Objective:To compare the therapeutic effects of Agkistrodon antivenomand Deinagkistrodon antivenom in patients bitten by Trimeresurus stejnegeri.Methods:Seventy-four patients with Trimeresurus stejnegeri bite admitted to Qianxinan Buyi and Miao Autonomous Prefecture People's Hospital from January 2023 to January 2025 were selected as the research objects.According to the random number table method,they were divided into the control group and the observation group,with 37 cases in each group.Both groups received conventional treatment.On this basis,the control group was treated with Agkistrodon antivenom,and the observation group was treated with Agkistrodon antivenom.Coagulation parameters,inflammatory markers,and complication rates were compared.Results:At 3 days post-treatment,both groups exhibited shortened prothrombin time and activated partial thromboplastin time,elevated fibrinogen levels,and reduced D-dimer levels,with the observation group demonstrating superior outcomes compared to the control group(P<0.05).At 3 days post-treatment,white blood cell count and C-reactive protein levels decreased in both groups,but were significantly lower in the observation group(P<0.05).The total complication rate in the observation group was markedly lower than that in the control group(P<0.001).Conclusion:Deinagkistrodon antivenom is superior to Agkistrodon antivenom in managing Trimeresurus stejnegeri bites,effectively restoring coagulation function,reducing systemic inflammation,and minimizing complications.
