摘　　要：目的:分析医联体模式下的社区心脏康复对老年冠心病(CHD)患者的影响。方法:选取2023年6月—2024年6月北京市朝阳区安贞社区卫生服务中心收治的70例老年CHD患者开展研究,随机分为对照组(35例,常规慢性病管理)和观察组(35例,基于医联体模式开展社区心脏康复)。比较两组心功能指标、运动耐力、心肌损伤指标、心血管事件和预后。结果:治疗后,两组N末端脑钠肽前体水平降低,左心室射血分数、左心室舒张末期内径升高,6 min步行试验距离延长,观察组优于对照组(P<0.05);治疗后,两组心肌肌钙蛋白I、肌酸激酶同工酶、肌红蛋白水平降低,观察组低于对照组(P<0.05);观察组心血管事件发生率、再住院率、再狭窄率均低于对照组(P<0.05)。结论:医联体模式下的社区心脏康复能有效增强老年CHD患者心功能、运动耐力,降低心血管事件发生率,改善预后。Objective:To evaluate the effects of community-based cardiac rehabilitation under a medical consortium model in elderly patients with coronary heart disease(CHD).Methods:Seventy elderly patients with coronary heart disease treated at Beijing Chaoyang District Anzhen Community Health Service Center from June 2023 to June 2024 were randomized to a control group(n=35,receiving routine chronic disease management)and an intervention group(n=35,receiving medical consortium-based cardiac rehabilitation).Cardiac function parameters,exercise capacity,myocardial injury index,cardiovascular events,and prognostic outcomes were compared.Results:After treatment,the levels of N-terminal pro-brain natriuretic peptide decreased in both groups,while left ventricular ejection fraction,left ventricular end-diastolic diameter,and 6-minute walk test distance increased,with the observation group showing superior outcomes compared to the control group(P<0.05).After treatment,the levels of cardiac troponin I,creatine kinase-MB,and myoglobin decreased in both groups,with the observation group exhibiting lower levels than the control group(P<0.05).The observation group also demonstrated lower rates of cardiovascular events,rehospitalization,and restenosis compared to the control group(P<0.05).Conclusion:Medical consortium-facilitated community cardiac rehabilitation significantly enhances cardiac recovery,functional capacity,and clinical outcomes in elderly patients with coronary heart disease.
