六君子汤联合布地奈德治疗老年肺脾气虚型慢性阻塞性肺疾病的临床效果观察

摘　　要：目的:观察六君子汤联合布地奈德治疗老年肺脾气虚型慢性阻塞性肺疾病(COPD)患者的效果。方法:选取天津市河西区中医医院呼吸内科2023年4月—2024年4月收治的90例老年肺脾气虚型COPD患者,按照抽签法分为对照组和联合组,各45例。对照组使用布地奈德治疗,联合组使用六君子汤联合布地奈德治疗。比较两组中医证候积分、肺功能、血气指标。结果:治疗后,两组中医证候积分降低,联合组低于对照组(P<0.001);治疗后,两组第1秒用力呼吸容积、用力肺活量、第1秒用力呼吸容积/用力肺活量水平升高,联合组高于对照组(P<0.05);治疗后,两组动脉血氧分压水平升高,动脉血二氧化碳分压水平降低,联合组优于对照组(P<0.05)。结论:六君子汤联合布地奈德治疗能够减轻老年肺脾气虚型COPD患者的临床症状,提高肺功能,改善血气指标。Objective:To observe the clinical efficacy of Liujunzi decoction combined with budesonide in treating elderly patients with chronic obstructive pulmonary disease(COPD)of lung-spleen qi deficiency pattern.Methods:Ninety elderly COPD patients with lung-spleen qi deficiency pattern admitted to the Department of Respiratory Medicine at Tianjin Hexi District Hospital of Traditional Chinese Medicine,from April 2023 to April 2024 were selected and randomly divided into a control group and a combination group by drawing lots,with 45 cases in each group.The control group received budesonide treatment,while the combination group received Liujunzi decoction combined with budesonide.Traditional Chinese medicine syndrome scores,lung function,and blood gas parameters were compared between the two groups.Results:After treatment,Traditional Chinese Medicine syndrome scores decreased in both groups,with the combination group showing lower scores than the control group(P<0.001).Post-treatment forced expiratory volume in the first second,forced vital capacity,and forced expiratory volume in the first second/forced vital capacity levels increased in both groups,with the combination group exhibiting higher values than the control group(P<0.05).Arterial partial pressure of oxygen levels increased,and arterial partial pressure of carbon dioxide levels decreased in both groups after treatment,the combination group was betten than the control group(P<0.05).Conclusion:The combination of Liujunzi decoction and budesonide alleviates clinical symptoms,improves lung function,and optimizes blood gas parameters in elderly COPD patients with lung-spleen qi deficiency pattern.
