督灸联合土蛭八珍汤对胃、结直肠癌术后化疗所致骨髓抑制的疗效

摘　　要：目的探讨督灸联合土蛭八珍汤对防治胃、结直肠癌术后化疗所致骨髓抑制的临床疗效。方法将2021年6月至2022年11月因胃、结直肠癌于河南省中医院(河南中医药大学第二附属医院)行术后辅助化疗的患者随机分为对照组(43例)和观察组(38例),对照组根据2021版中国临床肿瘤学会指导意见接受常规一线化学治疗,观察组在对照组基础上接受督灸联合土蛭八珍汤治疗。比较两组在6周期化疗过程中骨髓抑制程度、升白针应用次数、营养指标、卡氏功能状态评分(KPS)、体重指数(BMI)、对化疗方案的影响和末次化疗结束第7天化验指标[白细胞计数(WBC)、中性粒细胞计数(NEUT)、红细胞计数(RBC)、血小板计数(PLT)、淋巴细胞计数(LYMPH)、血红蛋白(HB)、白蛋白(ALB)、前白蛋白(PA)、视黄醇结合蛋白(RBP)]等方面的差异性,对可能影响骨髓抑制程度的相关危险因素行logistic回归分析。结果与对照组比较,观察组在化疗过程中骨髓抑制程度更低、升白针应用次数更少、对化疗方案的影响更小(P<0.05);与对照组比较,末次化疗结束第7天观察组的WBC、NEUT、LYMPH、ALB、PA、RBP、BMI、KPS评分等更高(P<0.05),但对照组与观察组的RBC、PLT和HB等差异无统计学意义(P>0.05);多元有序logistic回归分析显示肿瘤的临床分期越晚、BMI越低,化疗后发生骨髓抑制的程度越重(P<0.05)。结论督灸联合土蛭八珍汤对防治胃、结直肠癌术后化疗所致WBC和NEUT降低具有一定的临床效果,疾病分期和BMI是影响骨髓抑制程度的独立危险因素。Objective To investigate the clinical efficacy of Du-moxibustion combined with Tuzhi Bazhen decoction in the prevention and treatment of bone marrow suppression caused by chemotherapy after gastric and colorectal cancer surgery.Methods The patients who underwent postoperative adjuvant chemotherapy for gastric and colorectal cancer in Henan Province Hospital of Traditional Chinese Medicine(the Second Affiliated Hospital of Henan University of Traditional Chinese Medicine)from June 2021 to November 2022 were randomly divided into control group(43 cases)and observation group(38 cases).The control group was given routine first-line chemotherapy,while the observation group was given Du-moxibustion combined with Tuzhi bazhen decoction on the basis of the control group.The selection of chemotherapy regimen shall be carried out according to the guidance of the 2021 Chinese Society of Clinical Oncology(CSCO).First,the differences in the degree of bone marrow suppression,the number of times of application of drugs to increase leukocytes,nutritional indicators,Karnofsky performance status(KPS),body mass index(BMI),the impact on the chemotherapy regimen and the laboratory indicators[white blood cell count(WBC),neutrophil count(NEUT),red blood cell count(RBC),platelet count(PLT),lymphocyte count(LYMPH),hemoglobin(HB),albumin(ALB),prealbumin(PA),retinol binding protein(RBP)]on the 7th day after the end of the last chemotherapy were compared between the two groups during the 6-cycle chemotherapy,and finally logistic regression analysis was performed on the relevant risk factors that may affect the degree of bone marrow suppression.Results Compared with the control group,the observation group had lower degree of bone marrow suppression,fewer times of application of leukocyte-enhancing drugs and less influence on the chemotherapy regimen(P<0.05).On the 7th day after the end of 6 cycles of chemotherapy,the WBC,NEUT,LYMPH,ALB,PA,RBP,BMI and KPS score of the observation group were higher than those of the control group(P<0.05),but t
