摘  要:目的探讨增强型体外反搏(EECP)对不稳定型心绞痛(UA)合并肥胖患者睡眠及心功能的干预效果。方法选取2022年9月至2023年9月郑州大学第二附属医院心内科收治的160例UA合并肥胖的患者,对照组和EECP组按1∶1等比例随机分组,各80例。两组患者均接受常规的药物治疗和护理,干预组在常规药物治疗和护理的基础上联合EECP治疗,采用重复测量方差分析比较两组患者干预前、干预结束后、干预结束3个月后的睡眠呼吸暂停低通气指数(AHI)、日间嗜睡、睡眠效率、心指数(CI)、全身血管外周阻力(SRV)。结果与对照组比较,EECP治疗1个疗程后EECP组患者AHI、日间嗜睡、睡眠效率、CI、SRV均有改善(P<0.05);治疗结束3个月后,EECP组日间嗜睡、睡眠效率、CI、SRV均有改善(P<0.05)。结论体外反搏可改善UA合并肥胖患者睡眠及心功能,且疗效可持续3个月。Objective To investigate the effect of enhanced external counter pulsation(EECP)on sleep and cardiac function in patients with unstable angina(UA)pectoris complicated by obesity.Methods A total of 160 patients with unstable angina complicated with obesity admitted to the Department of Cardiology in the Second Affiliated Hospital of Zhengzhou University from September 2022 to September 2023 were selected and divided into control group(80 cases)and EECP group(80 cases)according to random number table method.Patients in both groups were given conventional drug treatment and nursing.The intervention group was combined with EECP on the basis of conventional drug treatment and nursing.The sleep apnea hypopnea index(AHI),daytime sleepiness,sleep efficiency,cardiac index(CI)and systemic vascular peripheral resistance(SRV)of the two groups were compared by repeated analysis of measurement variance before intervention,after intervention and 3 months after intervention.Results Compared with control group,AHI,daytime sleepiness,sleep efficiency,CI,SRV were improved in EECP group after 1 course of EECP treatment(P<0.05).Daytime sleepiness,sleep efficiency,CI and SRV were improved 3 months after EECP treatment(P<0.05).Conclusion Enhanced extracorporeal counterpulsation can improve sleep and cardiac function in obese patients with unstable angina pectoris,and the curative effect can last for 3 month.
