摘 要:目的分析血清巨噬细胞炎症蛋白-1α(MIP-1α)联合正五聚蛋白3(PTX3)检测在慢性肾衰竭血液透析患者并发细菌性肺炎中的诊断价值。方法选取2023年1月至2024年1月在平煤神马医疗集团总医院进行血液透析的110例慢性肾衰竭患者作为本次研究对象,根据住院期间是否并发细菌性肺炎,将其分为感染组(32例)和未感染组(78例)。采用酶联免疫吸附法检测两组血清MIP-1α、PTX3水平,采用logistic回归多元相关性分析并发细菌性肺炎的危险因素,进一步受试者工作特征(ROC)曲线分析血清MIP-1α、PTX3检测对慢性肾衰竭血液透析并发细菌性肺炎的诊断价值。结果感染组血清MIP-1α、PTX3水平均高于未感染组(P<0.05)。多因素logistic回归方程分析结果显示,血清MIP-1α与PTX3是慢性肾衰竭血液透析患者并发细菌性肺炎的独立危险因素。ROC曲线显示,血清MIP-1α、PTX3诊断慢性肾衰竭血液透析患者并发细菌性肺炎的曲线下面积(AUC)分别为0.782、0.812,联合诊断AUC为0.901。结论血清MIP-1α联合PTX3检测对慢性肾衰竭血液透析患者并发细菌性肺炎具有较高的诊断价值,可作为临床诊断该疾病的有效参考指标。Objective To analyze the diagnostic value of serum macrophage inflammatory protein-1α(MIP-1α)combined with pentameric protein 3(PTX3)detection in patients with chronic renal failure undergoing hemodialysis complicated with bacterial pneumonia.Methods A total of 110 patients with chronic kidney failure who underwent hemodialysis in General Hospital of Pingmei Shenma Group from January 2023 to January 2024 were selected as the study subjects.Based on whether they developed bacterial pneumonia during hospitalization,they were divided into an infected group(32 cases)and an uninfected group(78 cases).Enzyme linked immunosorbent assay was used to detect the levels of MIP-1αand PTX3 in two groups of serum.Logistic regression was used to analyze the risk factors for bacterial pneumonia.Furthermore,receiver operating characteristic(ROC)curve analysis was performed to evaluate the diagnostic value of serum MIP-1αand PTX3 detection for bacterial pneumonia in patients with chronic renal failure undergoing hemodialysis.Results The levels of serum MIP-1αand PTX3 in the infected group were higher than those in the uninfected group(P<0.05).Multivariate logistic regression analysis results showed that serum MIP-1αand PTX3 were independent risk factors for bacterial pneumonia in hemodialysis patients with chronic renal failure.The ROC curve showed that area under the curve(AUC)of serum MIP-1αand PTX3 for diagnosing bacterial pneumonia in hemodialysis patients with chronic renal failure were 0.782 and 0.812,respectively,and the AUC for combined diagnosis was 0.901.Conclusion The clinical diagnosis of bacterial pneumonia in patients with chronic renal failure undergoing hemodialysis has high diagnostic value for serum MIP-1αcombined with PTX3 detection,which can serve as an effective reference index for clinical diagnosis of this disease.
