摘  要:目的分析重症肺部感染患者外周血高迁移率族蛋白B1(HMGB1)、程序性死亡蛋白-1(PD-1)表达水平与免疫功能和心肌损害的关系。方法选取2020年9月至2022年9月平煤神马医疗集团总医院收治的235例肺部感染患者,分为重症肺部感染患者(重症组)84例和轻症肺部感染患者(轻症组)151例,同时选择同期体检的100例健康体检者为对照组。比较3组外周血HMGB1、PD-1表达水平、免疫功能指标水平以及心肌损害标志物水平,并采用Pearson相关检验分析重症肺部感染患者外周血HMGB1、PD-1表达水平与免疫功能和心肌损害的相关性。结果重症组外周血HMGB1、PD-1表达水平高于轻症组和对照组(P<0.05),且轻症组高于对照组(P<0.05);重症组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)低于轻症组和对照组(P<0.05),且轻症组低于对照组(P<0.05);重症组血清CK-MB、LDH、Mb、cTnⅠ水平高于轻症组和对照组(P<0.05),且轻症组高于对照组(P<0.05);Pearson相关分析结果显示,重症肺部感染患者外周血HMGB1、PD-1表达水平与CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、IgA、IgG、IgM均呈负相关(P<0.05),与肌酸激酶同工酶(CK-MB)、乳酸脱氢酶(LDH)、肌红蛋白(Mb)、心肌肌钙蛋白Ⅰ(cTnⅠ)均呈正相关(P<0.05)。结论重症肺部感染患者外周血HMGB1、PD-1表达水平显著升高,且二者表达水平与患者免疫功能和心肌损害密切相关,临床上可予以重点关注。Objective To analyze the relationship between expression levels of high-mobility group box 1(HMGB1)and programmed death protein-1(PD-1)in peripheral blood and immune function and myocardial damage in patients with severe pulmonary infection.Methods A total of 235 patients with pulmonary infection admitted to General Hospital of Pingmei Shenma Group from September 2020 to September 2022 were selected and divided into 84 cases in severe pulmonary infection(severe group)and 151 cases in mild pulmonary infection(mild group).A total of 100 healthy subjects who underwent physical examination during the same period were selected as control group.The expression levels of peripheral blood HMGB1 and PD-1,immune function indicators and myocardial damage markers were compared among the three groups,and Pearson correlation test was used to analyze the correlation of expression levels of HMGB1 and PD-1 in peripheral blood of patients with severe pulmonary infection with immune function and myocardial damage.Results The expression levels of peripheral blood HMGB1 and PD-1 in severe group were higher than those in mild group and control group(P<0.05),and the expression levels in mild group were higher than those in control group(P<0.05).The CD3^(+),CD4^(+),CD4^(+)/CD8^(+),immunoglobulin A(IgA),immunoglobulin G(IgG)and immunoglobulin M(IgM)were lower in severe group than those in mild group and control group(P<0.05),and the above indicators were lower in mild group than those in control group(P<0.05).The levels of serum creatine kinase-MB(CK-MB),lactatedehydrogenase(LDH),myoglobin(Mb)and cardiac troponinⅠ(cTnⅠ)in severe group were higher compared with those in mild group and control group(P<0.05),and the levels in mild group were higher than those in control group(P<0.05).Pearson correlation analysis showed that the expression levels of HMGB1 and PD-1 in peripheral blood of patients with severe pulmonary infection were negatively correlated with CD3^(+),CD4^(+),CD4^(+)/CD8^(+),IgA,IgG and IgM(P<0.05),and were positively corre
