双环法乳房整形术治疗乳房下垂患者的效果

摘　　要：目的探讨双环法乳房整形术治疗乳房下垂的效果。方法回顾性分析2020年1月至2022年1月就诊于新乡医学院第一附属医院的乳房下垂患者的临床资料,将接受双环法乳房整形术的30例纳入观察组,将接受垂直双蒂法的30例纳入对照组。比较两组的手术情况(手术时间、术中出血量、切口大小);并评估其术前、术后3个月、12个月时两组患者的乳房形态(乳房对称性、乳房基底宽度、乳房下皱襞中点-乳头距、胸骨-乳头距、锁骨-乳头距、乳房突度)和乳头乳晕感觉功能(乳晕直径、乳头乳晕感觉);评估术后3个月和12个月时两组患者的并发症(早期并发症、远期并发症)及满意度[乳腺癌患者报告结局量表(BC-PROM)]。结果观察组患者的手术情况优于对照组(P<0.05);术后3个月、12个月时,两组乳房形态和乳头乳晕感觉功能均较术前改善,且观察组优于同期对照组(P<0.05);观察组的总并发症发生率低于对照组(P<0.05);术后3个月、12个月时,观察组的乳房乳头满意度、对手术结局满意度以及患者获取信息满意度评分均高于对照组(P<0.05)。结论双环法乳房整形术对于乳房下垂患者具有明显的治疗效果,瘢痕隐蔽,可以获得持久的效果,提高患者满意度。Objective To explore the effect of double-ring mammoplasty for mastoptosis correction.Methods Clinical data of patients with mastoptosis treated in the First Affiliated Hospital of Xinxiang Medical University from January 2020 to January 2022 were retrospectively analyzed.Thirty cases receiving double-ring mammoplasty were included in observation group,and 30 cases receiving vertical double pedicle surgery were enrolled as control group.The surgical conditions(surgical time,intraoperative blood loss,incision size)of the two groups were compared.The breast shape(breast symmetry,breast base width,midpoint-papillary distance of inframammary fold,sternal-nipple distance,clavicular-nipple distance,breast protrusion)and nipple areola sensory function(areolar diameter,nipple areola sensation)in the two groups were evaluated before surgery and at 3 months and 12 months after surgery.Complications(early complications,late complications)and satisfaction[patient-reported outcome measure for patients with breast cancer scale(BC-PROM)]were evaluated at 3 and 12 months after surgery.Results The operation condition of observation group was better than control group(P<0.05).At 3 months and 12 months after surgery,the breast morphology and sensory function of nipple and areola were improved in both groups,and observation group was better than control group(P<0.05).The total complication rate of observation group was lower than that of control group(P<0.05).At 3 months and 12 months after surgery,the satisfaction of breast nipple,satisfaction of surgical outcome and satisfaction of patient obtaining information in observation group were higher than those in control group(P<0.05).Conclusion Double-ring mammoplasty has obvious therapeutic effect on patients with mastoptosis,and the scar is concealed,which can obtain lasting effect and improve patient satisfaction.
