摘　　要：目的分析256层螺旋CT冠状动脉成像(CTA)对冠心病经皮冠状动脉介入治疗(PCI)术后恢复情况的评估价值。方法选择2020年7月至2022年7月于郑州市第七人民医院影像科行PCI术治疗的98例冠心病患者为本研究对象,PCI术后6个月进行256层CTA检查,2周内行冠脉造影(CAG)检查,评价256层CTA和CAG的图像质量和支架内再狭窄(ISR)情况,Kappa一致性分析256层CTA对ISR的评估价值。结果256层CTA平均图像质量评分和ISR诊断结果与CAG比较,差异无统计学意义(P>0.05)。256层CTA诊断PCI术后ISR的灵敏度为100.00%,特异度为96.83%,准确率为97.96%,阳性预测值为94.59%,阴性预测值为100.00%,Kappa值为0.956。256层CTA诊断PCI术后管壁厚度<0.1 mm的灵敏度为84.44%,特异度为79.25%,准确率为81.63%,阳性预测值为77.55%,阴性预测值为85.71%,Kappa值为0.633。结论256层CTA图像质量佳,对冠心病PCI术后恢复情况的诊断效能高,可作为术后恢复情况评估的参考依据。Objective To analyze the evaluation value of 256-slice spiral CT angiography(CTA)for the recovery of coronary heart disease after percutaneous coronary intervention(PCI).Methods A total of 98 patients with coronary heart disease undergoing PCI in Imaging Department of Zhengzhou Seventh People’s Hospital were enrolled as the research objects between July 2020 and July 2022.All underwent 256-slice CTA at 6 months after PCI and coronary angiography(CAG)within 2 weeks.The images quality and in-stent restenosis(ISR)by 256-slice CTA and CAG were evaluated.The evaluation value of 256-slice CTA for ISR was analyzed by Kappa consistence test.Results There was no significant difference in average score of images quality or diagnostic results of ISR between 256-slice CTA and CAG(P>0.05).The sensitivity,specificity,accuracy,positive predictive value,negative predictive value and Kappa value of 256-slice CTA in the diagnosis of ISR after PCI were 100.00%,96.83%,97.96%,94.59%,100.00%and 0.956,respectively.The sensitivity,specificity,accuracy,positive predictive value,negative predictive value and Kappa value of 256-slice CTA in the diagnosis of tube wall thickness<0.1 mm after PCI were 84.44%,79.25%,81.63%,77.55%,85.71%and 0.633,respectively.Conclusion 256-slice CTA has good images quality and high diagnostic efficiency for the recovery of coronary heart disease after PCI,which can be applied as a reference to evaluate the recovery after PCI.
