摘 要:目的探讨行异基因造血干细胞移植(allo-HSCT)的重型β-地中海贫血(重型β地贫)患儿相位角(PA)与肌肉量的相关性。方法采用前瞻性研究设计,纳入2023年11月—2024年4月在我院接受allo-HSCT的88例重型β地贫患儿。在allo-HSCT预处理前及移植后1个月使用生物电阻抗(BIA)测量患儿的体成分指标。比较移植前后去脂体重(FFM)、去脂体重指数(FFMI)、骨骼肌(SMM)、骨骼肌指数(SMI)、瘦组织群(SLM)及相对四肢骨骼肌指数(RASM)的变化,并探讨PA与上述指标的相关性。结果allo-HSCT后患儿的FFM、FFMI、SMM、SMI、SLM及RASM均显著低于预处理前(均P<0.001);allo-HSCT后PA与上述指标均呈正相关(均P<0.05);线性回归分析显示,allo-HSCT后PA与FFMI、SMM、SMI、SLM及RASM存在显著的影响关系(均P<0.05)。结论重型β地贫患儿allo-HSCT后肌肉量显著降低,且与PA密切相关。PA可作为评价重型β地贫患儿肌肉量的有效指标。Objective To investigate the correlation between phase angle(PA)and muscle mass in children withβ-thalassemia major undergoing allogeneic haematopoietic stem cell transplantation(allo-HSCT).Methods This prospective study included 88 children withβ-thalassemia major who underwent allo-HSCT at our hospital from November 2023 to April 2024.Body composition indicators were measured using bioelectrical impedance analysis(BIA)before the allo-HSCT conditioning regimen and one month after allo-HSCT.Changes in Fat-Free Mass(FFM),Fat-Free Mass Index(FFMI),Skeletal Muscle Mass(SMM),Skeletal Muscle Index(SMI),Soft Lean Mass(SLM),and Relative Appendicular Skeletal Muscle Index(RASM)were compared,and correlations between PA and these indicators were explored.Results Post-allo-HSCT,the FFM,FFMI,SMM,SMI,SLM,and RASM values were significantly lower than those measured before receiving the allo-HSCT conditioning regimen(all P<0.001).Additionally,PA post-allo-HSCT was positively correlated with all these indicators(all P<0.05).Linear regression analyses showed significant relationships between post-allo-HSCT PA and FFMI、SMM、SMI、SLM,and RASM(all P<0.05).Conclusions In children withβ-thalassemia major,muscle mass significantly decreases following allo-HSCT compared to before receiving the conditioning regimen.This reduction in muscle mass is associated with PA.Moreover,PA can be utilized as a reliable index for evaluating muscle mass in the pediatricβ-thalassemia population.
