摘  要:目的:探讨MR定量磁化率成像(QSM)、R_(2)^(*)成像、快速T_(2)矫正多回波单体素波谱成像(HISTO-MRS)和双能量CT(DECT)等成像技术对铁定量的价值,并探讨不同脂肪含量对铁定量的影响。方法:制备30支不同含铁浓度(0.1~3.0 mg/mL,梯度0.1 mg/mL)水模为纯铁组;对另外3组共90支相应含铁浓度水模分别添加10%、20%和30%脂肪,制备得到铁-脂肪水模(轻脂组、中脂组和重脂组各30支)。分别应用QSM、R_(2)^(*)、HISTO-MRS和DECT技术对每组水模进行扫描,经后处理分别得到水模内的QSM、R_(2)^(*)、R_(2)和虚拟铁含量(VIC)值。应用Pearson和Spearman检验分析各参数值与实际铁浓度的相关性,应用单因素方差分析和Kruskal-Wallis检验分析添加不同脂肪含量后各项成像技术测得的铁浓度的差异。结果:QSM值与实际铁浓度在0.1~1.1 mg/mL时呈显著正相关(r=0.987,P<0.05);在添加不同含量脂肪后,纯铁组、轻脂组、中脂组和重脂组之间的差异均无统计学意义(均P>0.05)。R_(2)^(*)值与实际铁浓度在0.1~1.2 mg/mL时呈显著正相关(r=0.999,P<0.05);在添加不同含量脂肪后,轻脂组、中脂组和重脂组测得的铁浓度均显著高于纯铁组(P<0.05)。R_(2)值与实际铁浓度在0.1~0.9 mg/mL时呈显著正相关(r=0.952,P<0.05);在添加不同含量脂肪后,纯铁组、轻脂组、中脂组和重脂组之间的差异均无统计学意义(P>0.05)。VIC值与实际铁浓度在0.1~3.0 mg/mL时呈高度正相关(r=0.662,P<0.05),在1.0~3.0 mg/mL时相关性更显著(r=0.942,P<0.05);在添加不同含量脂肪后,轻脂组、中脂组和重脂组测得的VIC值均明显低于纯铁组(均P<0.05),且随脂肪含量的增加,VIC值逐渐降低。结论:在铁浓度较低(<1.1 mg/mL)时,MRI优于DECT,而铁与脂肪共存时,QSM技术和HISTO-MRS技术比R_(2)^(*)技术更准确,且QSM技术更有优势;在较高铁浓度(>1.1 mg/mL)时,DECT优于MRI,但其准确性受脂肪影响较大,可作为特殊情况下MRI的替代方式。Objective:To explore the value of different effects on iron quantification of MR quantitative susceptibility mapping(QSM),R_(2)^(*),high-speed T_(2) corrected multi-echo single voxel spectroscopy(HISTO-MRS)and dual-energy CT(DECT),and to evaluate the effect of different fat contents on iron quantification.Methods:Thirty iron-containing water phantom(concentration range of 0.1~3.0mg/mL with gradient of 0.1mg/mL)were prepared as the pure iron group,10%,20%and 30%fat were added to different concentrations of iron-containing water phantom,and another 90 iron-fat water phantom were prepared as the light fat group,medium fat group and heavy fat group.Each water phantom were scanned using QSM,R_(2)^(*),HISTO-MRS and DECT.The values of QSM,R_(2)^(*),R_(2) and virtual iron content(VIC)were obtained with different postprocessing methods respectively.Pearson and Spearman methods were used to analyze the correlation between each value and actual iron concentration.One-way ANOVA and Kruskal-Wallis test were used to analyze the difference of iron concentration measured by each imaging technique after adding different fat contents.Results:There was a significant positive correlation between QSM values and actual iron concentrations at 0.1~1.1mg/mL(r=0.987,P<0.05),and there were no statistically significant differences between the pure iron group,the light fat group,the medium fat group and the heavy fat group after adding different contents of fat(P>0.05 for all).There was a significant positive correlation between R_(2)^(*)values and actual iron concentrations at 0.1~1.2mg/mL(r=0.999,P<0.05),the measured iron concentrations in the light fat,medium fat,and heavy fat groups were significantly higher than those of pure iron group after adding different contents of fat(P<0.05 for all).There was a significant positive correlation between R_(2) values and actual iron concentrations at 0.1~0.9mg/mL(r=0.952,P<0.05),and there were no statistically significant differences between the pure iron group,the light fat group,the me-dium fat g
