摘 要:目的分析红细胞分布宽度/白蛋白比值(RAR)与2型糖尿病微血管并发症的关系。方法选取2022年6月至2024年3月收治的104例2型糖尿病患者,根据是否发生微血管并发症分为未合并组60例与合并组44例。所有纳入对象入院24 h内检测外周血红细胞分布宽度(RDW)、白蛋白(ALB)水平,并计算RAR;分析各组临床基本资料、生化指标及RAR;采用受试者工作特征(ROC)曲线评估RAR对2型糖尿病患者发生微血管并发症的诊断价值,采用多因素logistic回归分析2型糖尿病患者发生微血管并发症的影响因素。结果合并组2型糖尿病患者外周血白细胞计数、血尿酸、血尿素(Scr)、血胱抑素C(CysC)、RDW、RAR均高于未合并组,ALB低于未合并组(P<0.05,P<0.01)。ROC曲线分析结果显示:外周血RAR诊断2型糖尿病患者发生微血管并发症的曲线下面积高于RDW、ALB单一诊断。多因素logistic回归分析显示,病程、Scr、CysC、ALB、RAR均为2型糖尿病患者发生微血管并发症的影响因素(P<0.01)。结论RAR在2型糖尿病微血管并发症患者中异常升高,是2型糖尿病患者发生微血管并发症的影响因素,其对2型糖尿病患者微血管并发症的发生具有良好的预测价值。Objective To analyze the relationship between red blood cell distribution width(RDW)/albumin(ALB)ratio(RAR)and microvascular complications in patients with type 2 diabetes mellitus(T2DM).Methods A total of 104 patients with T2DM admitted from June 2022 to March 2024 were selected and divided into the uncombination group(n=60)and the combination group(n=44)according to the presence or absence of microvascular complications.RDW and ALB levels in peripheral blood of all enrolled subjects were detected within 24 h after admission,and RAR was calculated.The basic clinical data,biochemical indexes and RAR of each group were analyzed.Receiver operating characteristic(ROC)curve was used to evaluate the diagnostic value of RAR for microvascular complications in patients with T2DM.Multivariate logistic regression was used to analyze the influencing factors of microvascular complications in patients with T2DM.Results The white blood cell count,serum uric acid,blood urea(Scr),serum cystatin C(CysC),RDW and RAR in the combination group were higher than those in the uncombination group,while ALB was lower than that in the uncombination group(P<0.05,P<0.01).Results of ROC curve analysis showed that the area under the curve(AUC)of RAR in peripheral blood in the diagnosis of microvascular complications in T2DM patients was higher than that of RDW and ALB alone.Multivariate logistic regression analysis showed that the course of disease,Scr,CysC,ALB and RAR were the influencing factors of microvascular complications in patients with T2DM(P<0.01).Conclusion RAR is abnormally elevated in T2DM patients with microvascular complications,which is an influencing factor of the occurrence of microvascular complications in T2DM patients.In addition,it has a good predictive value for the occurrence of microvascular complications in T2DM patients.
