摘　　要：目的探究盆底超声对足月产妇盆底肌功能的评估价值,并分析影响足月产妇盆底肌功能的因素。方法选取2020年12月至2024年7月产后复查的足月产妇61例,根据产妇盆底肌收缩力分为收缩力不良组40例、收缩力良好组21例。所有产妇行盆底超声检查,比较2组临床资料和盆底超声检查参数(膀胱尿道后角、膀胱颈移动度),采用受试者工作特征(ROC)曲线分析盆底超声检查参数对于足月产妇盆底肌收缩力的评估价值,并行多因素logistic回归分析足月产妇盆底肌收缩力的影响因素。结果收缩力不良组年龄大于收缩力良好组,有糖尿病史、经产、经阴道分娩、会阴裂伤产妇占比高于收缩力良好组(P<0.05,P<0.01)。收缩力不良组Valsalva动作下膀胱尿道后角和膀胱颈移动度均大于收缩力良好组(P<0.01)。ROC曲线分析显示,Valsalva动作下膀胱尿道后角和膀胱颈移动度对足月产妇盆底肌收缩力联合评估价值更好。多因素logistic回归分析显示,年龄、糖尿病史、经产、经阴道分娩、会阴裂伤均为足月产妇盆底肌收缩力的危险因素(P<0.05,P<0.01)。结论盆底超声可评估足月产妇产后盆底肌功能;年龄、糖尿病史、经产、经阴道分娩、会阴裂伤可影响足月产妇产后盆底肌收缩力,对上述因素及时进行干预可降低足月产妇盆底肌功能障碍的发生风险。Objective To explore the value of pelvic floor ultrasound in the evaluation of pelvic floor muscle function in full-term parturients,and to analyze the factors affecting pelvic floor muscle function in full-term parturients.Methods A total of 61 full-term parturients who underwent postpartum follow-up from December 2020 to July 2024 were selected.According to the contractility of pelvic floor muscle,they were divided into the poor contractility group(n=40)and the good contractility group(n=21).All parturients underwent pelvic floor ultrasound examination.Clinical data and pelvic floor ultrasound parameters(posterior vesicourethral angle,bladder neck mobility)of the two groups were compared.Receiver operating characteristic(ROC)curve was used to analyze the evaluation value of pelvic floor ultrasound parameters on pelvic floor muscle contractility of full-term parturients,and multivariate logistic regression was performed to analyze the influencing factors of pelvic floor muscle contractility in full-term parturients.Results The age of the poor contractility group was higher than that of the good contractility group,and the proportion of women with diabetes history,labor,vaginal delivery and perineal laceration was higher than that of the good contractility group(P<0.05,P<0.01).The posterior vesicourethral angle and bladder neck mobility under Valsalva movement in the poor contractility group was greater than that in the good contractility group(P<0.01).ROC curve analysis showed that posterior vesicourethral angle and bladder neck mobility under Valsalva movement had a better value in evaluating the contractility of pelvic floor muscle in full-term parturients,and the efficiency of the combined evaluation was better.Multivariate logistic regression analysis showed that age,diabetes history,labor,vaginal delivery and perineal laceration were all risk factors for the contractility of pelvic floor muscle in full-term parturients(P<0.05,P<0.01).Conclusion Pelvic floor ultrasound can evaluate postpartum pelvic floor mu
