改良扬刺针法联合康复训练对梨状肌综合征患者髋关节功能及疼痛情况的影响

摘　　要：目的探究梨状肌综合征患者采用改良扬刺针法联合康复训练对髋关节功能及疼痛情况的影响。方法收集2020年12月至2021年12月就诊的梨状肌综合征患者180例,采用随机数字表法分为改良扬刺组90例和普通针刺组90例,改良扬刺组采用改良扬刺针法联合康复训练治疗,普通针刺组采用普通针刺法联合康复训练治疗。治疗后比较2组临床疗效,治疗前后髋关节功能、疼痛情况、临床症状积分及腓总神经电生理指标。结果改良扬刺组临床总有效率为95.6%(86/90)明显高于普通针刺组的82.2%(74/90),差异有统计学意义(P<0.01)。治疗后2组髋关节功能评分均升高,且改良扬刺组高于普通针刺组(P<0.05,P<0.01)。治疗后2组疼痛视觉模拟评分法评分、各项临床症状积分均下降,且改良扬刺组明显低于普通针刺组(P<0.05,P<0.01)。治疗后,2组病变肢体腓总神经运动神经传导速度提高、潜伏期缩短,且改良扬刺组病变肢体腓总神经运动神经传导速度明显高于普通针刺组,潜伏期明显短于普通针刺组(P<0.05,P<0.01)。结论改良扬刺针法联合康复训练能改善梨状肌综合征患者髋关节功能、临床症状、腓总神经运动神经传导速度和潜伏期,降低患者疼痛程度,提升临床治疗效果。Objective To explore the effects of modified Yang-needling combined with rehabilitation training on hip function and pain in patients with piriformis syndrome.Methods A total of 180 patients with piriformis syndrome treated from December 2020 to December 2021 were collected and divided into modified Yang-needling group(n=90)and ordinary acupuncture group(n=90)by random number table method.The modified Yang-needling group was treated with modified Yang-needling combined with rehabilitation training,while the ordinary acupuncture group was treated with ordinary acupuncture combined with rehabilitation training.The clinical efficacy,hip function,pain,clinical symptom score and electrophysiological indexes of common peroneal nerve before and after treatment were compared between the two groups.Results The total clinical effective rate of modified Yang-needling group was 95.6%(86/90),which was significantly higher than that of ordinary acupuncture group(82.2%,74/90),and the difference was statistically significant(P<0.01).After treatment,the hip function score of the two groups was increased,which was higher in the modified Yang-needling group than in the ordinary acupuncture group(P<0.05,P<0.01).After treatment,the scores of visual analogue score and all clinical symptoms of the two groups were decreased,which were significantly lower in the modified Yang-needling group than in the ordinary acupuncture group(P<0.05,P<0.01).After treatment,the motor nerve conduction velocity of common peroneal nerve in the diseased limbs was increased and the latency was shortened in the two groups;the motor nerve conduction velocity of common peroneal nerve in the modified Yang-needling group was significantly higher than that in the ordinary acupuncture group,and the latency was significantly shorter than that in the ordinary acupuncture group(P<0.05,P<0.01).Conclusion Modified Yang-needling combined with rehabilitation training can improve the hip function and clinical symptoms of patients with piriformis syndrome,improve the motor
