摘　　要：目的探讨血清多配体蛋白聚糖-4(SDC-4)对类风湿关节炎(RA)患者的诊断价值。方法RA患者、系统性红斑狼疮(SLE)、强直性脊柱炎(AS)和干燥综合征(SS)患者以及该院同期健康体检者。检测血清SDC-4、抗环瓜氨酸多肽抗体(anti-CCP)、抗突变型瓜氨酸波形蛋白抗体(anti-MCV)和类风湿因子(RF)。结果RA组血清4项指标水平均显著高于其他对照组(P<0.01)。血清4项指标诊断RA的AUC均为中等诊断价值,其中anti-MCV最高。SDC-4检测在RA患者中的临床评价灵敏度和特异度分别为80.14%和78.90%,具有良好的诊断价值。4种血清标志物串并联分析结果显示串联分析特异度最大,为99.99%;并联分析SDC-4/anti-CCP/anti-MCV/RF的灵敏度最高,为99.83%。结论检测血清SDC-4水平对提高RA的诊断具有重要意义,且联合诊断的价值远大于单独检测。Objective To investigate the diagnostic value of serum syndecan-4(SDC-4)in patients with rheumatoid arthritis(RA).Methods Patients with RA,systemic lupus erythematosus(SLE),ankylosing spondylitis(AS)and dry syndrome(SS),and concurrent health check-ups at the institution.Serum SDC-4,anti-cyclic citrullinated polypeptide antibody(anti-CCP),anti-mutant citrullinated vimentin antibody(anti-MCV)and rheumatoid factor(RF)were tested.Results The levels of all 4 serum indicators were significantly higher in RA group than in other control groups(P<0.01).The AUC of all 4 serum indicators for the diagnosis of RA was of moderate diagnostic value,with anti-MCV being the highest.The sensitivity and specificity of SDC-4 assay in clinical evaluation of RA patients were 80.14% and 78.90%,respectively,which had good diagnostic value.The results of the tandem analysis of the four serum markers showed the highest specificity of 99.99% for the tandem analysis,and the highest sensitivity of 99.83% for both SDC-4/anti-MCV and anti-MCV and RF for the tandem analysis.Conclusion The detection of serum SDC-4 levels in RA patients was important to improve the diagnosis of the disease,and the value of the combined diagnosis was much greater than that of the individual tests,which helps in the diagnosis of RA patients.
