摘 要:目的分析0~14岁儿童水痘-带状疱疹病毒(Varicella-zoster virus,VZV)抗体水平及其影响因素。方法收集江苏省2021-2023年免疫水平监测项目中扬州市宝应县0~14岁儿童的剩余血清标本,使用酶联免疫吸附试验检测VZV免疫球蛋白(Immunoglobulin G,IgG)抗体,分析抗体阳性率、几何平均浓度(Geometric mean concentration,GMC)及其影响因素。结果共纳入研究对象409例,VZV IgG抗体总阳性率、GMC分别为71.6%(95%CI:67.1%~75.8%)、375.5(95%CI:326.1~424.9)mIU/mL。多因素分析显示,城市地区抗体阳性率高于城乡结合区(OR=1.77,95%CI:1.12~2.80),接种1剂次和2剂次水痘疫苗(Varicella vaccine,VarV)高于未接种VarV(OR=3.51,95%CI:1.76~7.00;OR=4.93,95%CI:2.24~10.86);女性GMC高于男性(β=129.63,95%CI:32.38~226.88),城市地区高于城乡结合区(β=188.31,95%CI:93.34~283.28)。结论调查地区0~14儿童VZV抗体水平总体较高但存在地区差异,VarV接种显著提高了儿童抗体水平。Objective To analyze varicella-zoster virus(VZV)antibody levels in children 0-14 years of age and factors influencing antibody level.Methods We obtained residual sera from 0-14-year-old children in Baoying county of Yangzhou city from an immunization surveillance project that was conducted during 2021-2023 in Jiangsu province.We tested the sera for VZV immunoglobulin G(IgG)antibody using enzyme-linked immunosorbent assay to determine positivity rates and geometric mean concentrations(GMC)and their influencing factors.Results There were 409 children included in the study.The overall positivity rate was 71.6%(95%CI:67.1%to 75.8%)and the VZV IgG antibody GMC was 375.5 mIU/mL(95%CI:326.1 to 424.9).Multivariate analyses showed that the positivity rate was higher in urban areas than in urban-rural areas(OR=1.77,95%CI:1.12 to 2.80)and in children who had received one(OR=3.51,95%CI:1.76 to 7.00)or two(OR=4.93,95%CI:2.24 to 10.86)doses of varicella vaccine than in children who had not received varicella vaccine;VZV GMC was higher in girls than boys(β=129.63,95%CI:32.38 to 226.88)and in urban areas than urban-rural areas(β=188.31,95%CI:93.34 to 283.28).Conclusions VZV antibody levels were high among children 0-14 years of age in the surveyed area,although with variation by area.Varicella vaccination significantly increased VZV antibody levels in children.
