外周血单核细胞内STAT3表达与成人哮喘患者CysLT1受体拮抗剂治疗效果的关系

摘　　要：目的探讨外周血单核细胞内信号传导及转录激活因子3(signal transducer and activator of transcription 3,STAT3)表达与成人哮喘患者半胱氨酰白三烯(cysteinyl leukotrienes,CysLT1)受体拮抗剂治疗效果的关系。方法采用前瞻性分析,纳入2021年10月—2023年10月开封市人民医院收治的154例成人哮喘患者,所有患者均采用孟鲁司特治疗1周,根据治疗效果将其分为治疗有效组和治疗无效组。比较两组基线资料及治疗前外周血单核细胞内STAT3表达,采用相关性分析治疗前外周血单核细胞内STAT3表达与治疗效果的关系,并用logistic回归分析CysLT1受体拮抗剂治疗效果的影响因素,绘制受试者工作特征(receiver operating characteristic curve,ROC),以曲线下面积(area under curve,AUC)检验STAT3对CysLT1受体拮抗剂治疗效果的预测价值。结果154例患者中有139例患者治疗有效,有效率90.26%(139/154),15例患者治疗无效,无效率9.74%(15/154)。治疗无效组与治疗有效组有吸烟史占比[93.33%(14/15)比63.31%(88/139)]、LTE4[(170.16±32.22)ng/L vs(224.12±38.08)ng/L]、STAT3[(1.40±0.20)vs(1.22±0.12)]比较,差异均有统计学意义(P<0.05)。经点二列相关性分析,外周血单核细胞内STAT3表达与CysLT1受体拮抗剂治疗效果呈正相关(r=0.357,P<0.001);经logistic回归分析,吸烟史、LTE4、STAT3均为成人哮喘患者CysLT1受体拮抗剂治疗效果的影响因素(P<0.05)。绘制ROC曲线显示,STAT3对成人哮喘患者CysLT1受体拮抗剂治疗无效有一定预测价值(AUC=0.800,P<0.001)。结论外周血单核细胞内STAT3表达是成人哮喘患者CysLT1受体拮抗剂治疗效果的影响因素,且能预测患者治疗无效风险。Objective To explore the relationship between the expression of signal transducer and activator of transcrip-tion 3(STAT3)in peripheral blood mononuclear cells and the therapeutic effect of cysteinyl leukotrienes(CysLT1)re-ceptor antagonists in adult asthma patients.Methods A prospective analysis was conducted on 154 adult asthma pa-tients admitted to the hospital from October 2021 to October 2023.All patients were treated with montelukast for one week and were divided into a effective treatment group and a ineffective treatment group based on the treatment effect.The baseline data and the expression of STAT3 in peripheral blood mononuclear cells before treatment were compared be-tween the two groups.The relationship between the expression of STAT3 in peripheral blood mononuclear cells before treatment and the therapeutic effect was analyzed by correlation analysis.Logistic regression was used to analyze the in-fluencing factors of the therapeutic effect of CysLT1 receptor antagonists,and the receiver operating characteristic curve(ROC)was drawn.The area under curve(AUC)was used to test the predictive value of STAT3 for the therapeutic effect of CysLT1 receptor antagonists.Results Among the 154 patients,139 patients showed effective treatment with an effective rate of 90.26%(139/154),while 15 patients showed ineffective treatment with an ineffective rate of 9.74%(15/154).The proportion of smoking history between the ineffective treatment group and the effective treatment group was 93.33%(14/15)and 63.31%(88/139),LTE4[(170.16±32.22)ng/L vs(224.12±38.08)ng/L],and STAT3[(1.40±0.20)vs(1.22±0.12)],with a statistically significant difference(P<0.05).According to the two column correlation analysis,the expression of STAT3 in peripheral blood monocytes was positively correlated with the therapeutic effect of CysLT1 receptor antagonists(r=0.357,P<0.001).Logistic regression analysis showed that smoking history,LTE4,and STAT3 were all influencing factors for the therapeutic effect of CysLTI receptor antagonists in adult
