窝沟封闭联合氟保护预防儿童龋齿的效果

摘　　要：目的探讨分析窝沟封闭术与氟保护漆联合使用对儿童龋齿的预防效果,为儿童龋齿防治提供科学指导意见。方法利用医院电子信息系统导出2022年在永煤集团总医院口腔科进行口腔体检的儿童,从中随机选取128例儿童作为研究对象,通过随机数字表法将其分为2组(n=64)。对照组实施窝沟封闭术干预措施,试验组实施氟保护漆联合窝沟封闭术干预措施。跟踪随访1年,比较、分析两组研究对象的窝沟封闭剂保存情况、龋齿发生情况、牙周健康情况。结果随访6个月、12个月发现,试验组窝沟封闭剂保存完整率为96.88%(62/64)、93.75%(60/64),优于对照组保存完整率87.50%(56/64)、78.13%(50/64),差异具有统计学意义(χ^(2)=3.905、6.465,P<0.05);试验组龋齿患病率为0%(0/64)、9.38%(6/64),低于对照组龋齿患病率12.50%(8/64)、23.44%(15/64),差异具有统计学意义(χ^(2)=6.533、4.614,P<0.05);试验组龈沟出血指数(girgival bleeding index,GBI)和菌斑指数(plaque index,PLI)评分均低于对照组,试验组牙周健康情况优于对照组,差异具有统计学意义(Z=-2.619、-2.278,P<0.05)。结论窝沟封闭术联合氟保护漆能更好预防儿童龋齿,更有利于牙周健康,封闭剂保存更完整,值得临床推广使用。Objective To discuss and analyze the preventive effects of pit and fissure sealants combined with fluoride varnish on childhood dental caries,providing scientific guidance for the prevention and treatment of childhood caries.Methods Using the hospitals electronic information system,we extracted children who underwent oral examinations in the Department of Stomatology at Yongmei Group General Hospital in 2022.From this group,128 children were randomly selected as study subjects and divided into two groups(n=64)using a random number table method.The control group received pit and fissure sealant intervention,while the experimental group received fluoride varnish combined with pit and fissure sealant intervention.Follow-up was conducted for one year to compare and analyze the retention of pit and fissure sealants,incidence of dental caries,and periodontal health between the two groups.Results At the 6-month and 12-month follow-up,the experimental group showed a pit and fissure sealant retention rate of 96.88%(62/64)and 93.75%(60/64)respectively,which was superior to the control groups retention rates of 87.50%(56/64)and 78.13%(50/64).The diferences were statistically significant(χ^(2)=3.905,6.465;P<0.05).The caries incidence in the exper-imental group was 0%(0/64)and 9.38%(6/64)at 6 months and 12 months respectively,which was lower than the con-trol groups caries incidence rates of 12.50%(8/64)and 23.44%(15/64).These differences were statistically significant(χ^(2)=6.533,4.614;P<0.05).Additionally,the experimental group exhibited lower girgival bleeding index and plaque index scores compared to the control group,indicating better periodontal health.These differences were statistically significant(Z=-2.619,-2.278;P<0.05).Conclusion The combination of pit and fissure sealants with fluoride varnish is more effective in preventing dental caries in children and is beneficial for periodontal health.The sealants also demonstrate better retention rates,indicating they are worthy of clinical promotion and use.
