基于“阳化气,阴成形”论治多系统萎缩

摘　　要：多系统萎缩(multiple system atrophy, MSA)是一种罕见且致命的神经系统变性病,以自主神经功能障碍、多巴低反应的帕金森综合征和小脑综合征多种组合为特征。阳化气匮乏和阴成形不足是MSA发生的关键病机,阴阳俱虚是其迁延和发展的重要因素。治疗应以阴阳双补为法,自拟温肾健脑方温补肾阳、益精填髓,兼顾护正气,使阴阳调和、脑髓得养。结合临床病案对基于“阳化气,阴成形”论治MSA的诊疗思路及用药经验进行介绍。Multiple system atrophy(MSA) is a rare and fatal neurodegenerative disorder characterized by autonomic dysfunction, Parkinsonism with poor response to dopamine, and cerebellar symptoms. The key pathogenic mechanisms of MSA are the depletion of yang transforming into qi and the insufficiency of yin forming substance. The deficiency of both yin and yang is a significant factor in the disease's progression and development. Treatment should focus on the dual supplementation of yin and yang. To do so, Wenshen Jiannao Formula(Kidney-Warming and Brain-Fortifying Formula), an original prescription, was proposed, which could tonify kidney yang, benefit essence, and nourish the brain. In addition, it is essential to preserve the body's vital qi to harmonize yin and yang, ensuring nourishment of the brain marrow. This paper introduces the diagnostic and therapeutic approach to MSA based on the concept of “yang transforms into qi, while yin constitutes form, ” along with medication experiences, supported by clinical case studies.
