鼻内镜下经中鼻道联合泪前隐窝进路治疗复发性鼻腔-鼻窦内翻性乳头状瘤的临床效果观察

摘　　要：目的探讨予以复发性鼻腔-鼻窦内翻性乳头状瘤(SINP)患者鼻内镜下经中鼻道联合泪前隐窝进路治疗的效果。方法述选医院在2020年5月-2022年5月期间收治的70例复发性SINP患者为研究主体,根据收治顺序分成参照组(n=35)、观察组(n=35),予以参照组鼻内窥镜下经中鼻道肿瘤切除联合尖牙窝进路上颌窦肿瘤切除术治疗,予以观察组鼻内镜下经中鼻道联合泪前隐窝进路治疗,比较组间治疗效果、手术情况、鼻窦功能、并发症情况。结果(1)观察组治疗总有效率较参照组高,组间差异显著(P<0.05);(2)观察组手术时间、住院时间较参照组短,术中出血量较参照组少,组间差异显著(P<0.05);(3)观察组SNOT-20较参照组短,组间差异显著(P<0.05);(4)观察组术后并发症率较参照组低,组间差异显著(P<0.05)。结论予以复发性SINP患者鼻内镜下经中鼻道联合泪前隐窝进路治疗的效果显著,具有出血少、手术时间短、并发症少、恢复快等优势,可促进鼻窦功能恢复,值得推广。Objective To investigate the effect of nasal endoscopic transnasal passage combined with anterior lacrimal recess in patients with recurrent SINP(SINP).Method Seventy patients with recurrent SINP admitted to the hospital from May 2020 to May 2022 were selected as the main body of the study,and were divided into the reference group(n=35)and the observation group(n=35)according to the order of admission.The reference group was treated with endoscopic resection of tumor through the middle nasal canal combined with fossa approach resection of maxillary sinus tumor.The observation group was treated through the middle nasal passage combined with the anterior lacrimal recess under nasal endoscope,and the treatment effect,operation status,sinus function and complications were compared between the groups.Results(l)The total effective rate of observation group was higher than that of reference group,and the difference between groups was significant(P<0.05);(2)The operation time and hospital stay in the observation group were shorter than those in the reference group,and the amount of intraoperative blood loss was less than that in the reference group,with significant differences between the two groups(P<0.05);(3)The SNOT-20 in observation group was shorter than that in reference group,and the difference between groups was significant(P<0.05);(4)The postoperative complication rate of the observation group was lower than that of the reference group,and the difference between groups was significant(P<0.05).Conclusion The treatment of recurrent SINP patients with nasal endoscope through the middle nasal passage combined with the anterior lacrimal recess has significant effects,with less bleeding,short operation time,fewer complications,quick recovery and other advantages,which can promote the recovery of sinus function,and is worth promoting.
