奥美拉唑与泮托拉唑四联疗法对Hp感染胃溃疡患者炎症免疫及胃肠激素调节作用的影响

摘　　要：目的:观察奥美拉唑与泮托拉唑四联疗法对幽门螺杆菌(Hp)感染胃溃疡患者炎症免疫及胃肠激素调节作用的影响。方法:采用随机数字表法将我院2023年1月至2023年12月期间收治的80例Hp感染胃溃疡患者分为对照组(40例,奥美拉唑四联疗法)和研究组(40例,泮托拉唑四联疗法)。对比两组有效率、Hp根治率、炎症反应指标[白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)、白细胞介素-1(IL-1)、肿瘤坏死因子-α(TNF-α)]、免疫指标[CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)]、胃肠激素[生长抑素(SS)、胆囊收缩素(CCK)、胃泌素(GAS)、胃动素(MTL)]、不良反应发生率和复发率。结果:研究组的有效率、Hp根治率高于对照组(P<0.05)。两组治疗后hs-CRP、IL-1、IL-6、TNF-α下降,研究组低于对照组同时间点(P<0.05)。两组治疗后CD8^(+)下降,研究组低于对照组同时间点;CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)升高,研究组高于对照组同时间点(P<0.05)。两组治疗后CCK、GAS、MTL下降,研究组低于对照组同时间点;SS升高,研究组高于对照组同时间点(P<0.05)。两组复发率、不良反应发生率组间对比未见差异(P>0.05)。结论:与奥美拉唑四联疗法相比,泮托拉唑四联疗法治疗Hp感染胃溃疡,可提高Hp根除率和疗效,可能与调节炎症、免疫水平及胃肠激素有关,安全有效。Objective:To observe the effects of quadruple therapy with omeprazole and pantoprazole on inflammatory immunity,and gastrointestinal hormone regulation in patients with helicobacter pylor(Hp)infected gastric ulcer.Methods:Used random number table method,80 patients with Hp infected gastric ulcer who were admitted to our hospital from January 2023 to December 2023 were divided into control group(40 cases,quadruple therapy with omeprazole)and study group(40 cases,quadruple therapy with pantoprazole).The effective rate,Hp cure rate,inflammatory response indicators[interleukin-6(IL-6),high sensitivity-C reactive protein(hs-CRP),interleukin-1(IL-1),tumor necrosis factor-α(TNF-α)],immune indicators[CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+)],gastrointestinal hormones[somatostatin(SS),cholecystokinin(CCK),gastrin(GAS),motilin(MTL)],incidence of adverse reactions and recurrence rate between two groups were compared.Results:The effective rate and Hp cure rate of the study group were higher than those of the control group(P<0.05).hs-CRP,IL-1,IL-6 and TNF-αafter treatment decreased in both groups,the study group was lower than that of the control group at the same time point(P<0.05).CD8^(+)after treatment decreased in both groups,the study group was lower than that of the control group at the same time point,CD3^(+),CD4^(+),CD4^(+)/CD8^(+)increased,the study group was higher than that of the control group at the same time point(P<0.05).CCK,GAS MTL after treatment decreased in both groups,the study group was lower than that of the control group at the same time point,SS increased,the study group was higher than that of the control group at the same time point(P<0.05).There was no significant difference in the recurrence rate and incidence of adverse reactions between the two groups(P>0.05).Conclusion:Compared with quadruple therapy with omeprazole,quadruple therapy with pantoprazole in patients with Hp infected gastric ulcers,can improve the eradication rate and Hp cure rate,which may be related to regulating inflammation,im
