摘　　要：目的:探讨高迁移率组蛋白1(HMGB1)联合白细胞分化抗原(CD)38、CD138阳性对慢性子宫内膜炎(CE)的诊断价值。方法:选取2022年11月至2024年2月我院收治的152例行宫腔镜检查的患者,取子宫内膜组织,应用免疫组化法检测HMGB1、CD38、CD138表达。以苏木精-伊红(HE)染色作为CE诊断的金标准。Kappa检验HMGB1、CD38、CD138以及联合诊断与HE染色结果的一致性。受试者工作特征(ROC)曲线分析HMGB1、CD38、CD138单独以及联合诊断CE的价值。结果:152例行宫腔镜检查患者中,发生CE患者86例,未发生CE患者66例。CE组子宫内膜组织中HMGB1、CD38、CD138阳性表达率均高于非CE组(P<0.05)。Kappa检验分析HMGB1、CD38、CD138单独检测的免疫组织化学结果与HE染色结果的一致性中等(Kappa值=0.551、0.486、0.495,P<0.05),联合检测一致性较高(Kappa值=0.853,P<0.05)。HMGB1、CD38、CD138单独及联合诊断CE的曲线下面积(AUC)分别为0.779、0.747、0.756、0.927,且三指标联合检测的诊断效能高于单一指标检测的诊断效能(Z=5.121、5.759、5.901,P<0.05)。结论:HMGB1、CD38、CD138联合检测可提高对CE的诊断效能。Objective:To investigate the diagnostic value of high mobility histone 1(HMGB1)combined with leukocyte differentiation antigen(CD)38 and CD138 positive in chronic endometritis(CE).Methods:152 women who underwent hysteroscopy in our hospital from November 2022 to February 2024 were selected,hysteroscopic biopsy tissues were taken,and HMGB1,CD38 and CD138were detected by immunohistochemistry expression.HE staining was used as the gold standard for CE diagnosis.The consistency of HMGB1,CD38,CD138 and their combined diagnosis with HE staining results were tested by Kappa.The value of HMGB1,CD38,CD138 and combined diagnosis of CE were analyzed by receiver operating characteristic(ROC)curve.Results:Among the 152 patients who underwent hysteroscopy examination,86 had CE and 66 did not.The positive expression rates of HMGB1,CD38,and CD138 in the endometrial tissue of the CE group were higher than those in the non CE group(P<0.05).Kappa test analysis showed immunohistochemical results of individual detection of HMGB1,CD38,CD138 with HE staining results had moderate consistency(Kappa value=0.551,0.486,0.495,P<0.05),the consistency of joint detection was high(Kappa value=0.853,P<0.05).The area under the curve(AUC)of HMGB1,CD38,and CD138 for the diagnosis of CE alone and in combination were 0.779,0.747,0.756,and 0.927,respectively,and the diagnostic efficacy of the combined detection of the three indicators was higher than that of the single indicator detection(Z=5.121,5.759,5.901,P<0.05).Conclusion:The combined detection of HMGB1,CD38,and CD138 can improve the diagnostic efficiency of CE.
