青蒿素及其衍生物对肾损伤的干预作用及机制研究进展

摘　　要：肾脏疾病是由损伤、感染、中毒等原因导致肾单位丢失的一种疾病,其严重程度可危及患者生命。中药青蒿具有清虚热、除骨蒸、解暑热、截疟、凉血等功效。青蒿素是从中药青蒿中提取的一种含有过氧桥结构的倍半萜类化合物,现代研究发现青蒿素及其衍生物除具有强大的抗疟作用外,还具有抗炎、免疫调节、抗组织纤维化等药理作用。随着研究的不断深入,发现青蒿素及其衍生物在肾脏疾病领域有着独特的作用。为此,本文作者通过查阅归纳近10年文献,就青蒿素及其衍生物在肾病中的应用进行综述,分别对青蒿素及其衍生物干预急性肾损伤(缺血再灌注损伤、程序性死亡型急性肾损伤、药物性肾损伤)、慢性肾病(狼疮性肾炎、免疫球蛋白A肾病、糖尿病肾病、肾病综合征、肾纤维化)的治疗作用、分子机制、以及临床应用现状进行归纳总结,以期为青蒿素类药物的后续研究提供新颖见解,并对临床应用提供相关理论依据。Kidney disease is a disorder caused by inflammation,renal fibrosis,cell pyroptosis and other causes which lead to the loss of renal unit.In several cases,it can even endanger the lives of patients.The traditional Chinese medicine Artemisia annua has the effects of clearing heat,removing bone steaming,relieving summer heat,intercepting malaria,cooling blood and so on.Artemisinin extracted from the plant Artemisia annua L.,is a sesquiterpene compound containing a peroxy-bridge structure which extracted from the plant Artemisia annua L..Modern studies have found that artemisinin and its derivatives not only have strong antimalarial effects,but also have anti-inflammatory,immune regulation,anti-tissue fibrosis and other pharmacological effects.With the deepening of research,it has been found that artemisinin and its derivatives play a unique role in the field of kidney disease.In this paper,the application of artemisinin and its derivatives in nephropathy was reviewed.This article reviews the application of artemisinin and its derivatives in nephropathy by reviewing literatures in the past 10 years.The therapeutic effect,molecular mechanism and clinical application of artemisinin and its derivatives on acute kidney injury(programmed death acute kidney injury,ischemia-reperfusion injury,drug-induced kidney injury),chronic kidney disease(lupus nephritis,immunoglobulin A nephropathy,diabetic nephropathy,renal fibrosis) were analyzed and summarized,in order to provide relevant basis for further research and clinical application of artemisinin drugs.
