摘  要:目的观察胶原酶软膏对深Ⅱ度烫伤大鼠模型创面修复情况,并探究其是否具有抗炎、促愈合、清创、脱痂等作用,以及对于大鼠创面血管新生的影响。方法选取雄性SD大鼠,随机编号。建立3种深Ⅱ度大鼠损伤模型(高温烫伤模型、化学灼伤模型及热烧伤模型),每个模型分为5组,即模型组(MODEL组)、湿润烧伤膏组(MEBO组)、胶原酶一天一次组(COL 1D/T组)、胶原酶两天一次组(COL 2D/T组)及胶原酶两天一次并注射抗生素(左氧氟沙星)组(COL 2D/T+LEV组),另有10只大鼠作为空白对照组。HE染色切片观察是否造模成功及药物对各模型组织病理形态的影响。免疫组化试验检测血管内皮细胞标记物血小板-内皮细胞黏附分子(platelet endothelial cell adhesion molecule-1,CD31)及创面生长因子:碱性成纤维细胞生长因子(basic fibroblast growth factor,bFGF)、表皮生长因子(epidermal growth factor,EGF)的表达情况。ELISA试剂盒检测血清中炎症相关因子:肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、肿瘤坏死因子α刺激基因/诱导蛋白-6(tumor necrosis factor alpha stimulated gene/inducible protein 6,TSG-6)和白细胞介素-6(interleukin 6,IL-6)的含量。结果3种烫伤模型中,与MODEL组相比,MEBO组和COL 1D/T组可以明显缩短创面愈合时间并使创面微血管新生数目显著增加;同时,检测结果显示,MEBO组和COL 1D/T组可有效降低烫伤大鼠血清中炎性因子TNF-α、TSG-6和IL-6含量并显著提高创面因子bFGF和EGF含量。结论COL 1D/T组相比较于其他时长给药组对深Ⅱ度烫伤创面具有显著抗炎和促愈合作用。胶原酶作用于深Ⅱ度烫伤创面的机制与加速坏死组织分离、增加微血管数目、减轻局部炎性反应和增加创面bFGF和EGF的表达有关。Objective To observe the effect of collagenase ointment on wound repair in a rat model of deep second-degree scald,and to explore whether it has the effects of anti-inflammatory,pro-healing,debridement,and scab removal,as well as its effect on angiogenesis of the wound in rats.Methods Male SD rats were selected and numbered randomly.Three kinds of deep second degree rat injury models(high temperature scald model,chemical burn model and thermal burn model) were established,each model was divided into 5 groups,namely model group(MODEL),MEBO group(MEBO),collagenase once a day group(COL 1D/T),collagenase once two days group(COL 2D/T) and collagenase once two days and antibiotic(levofloxacin) injection group(COL 2D/T+LEV);another 10 rats were used as blank control group.The HE stained sections were used to observe whether the modeling was successful and the effects of drugs on the pathological morphology of each model.Immunohistochemical tests were performed to detect the expression of vascular endothelial cell marker platelet endothelial cell adhesion molecule-1(CD31) and wound growth factors basic fibroblast growth factor(bFGF) and epidermal growth factor(EGF).Elisa kit was used to detect inflammation-related factors in the serum:tumor necrosis factor-α(TNF-α),tumor necrosis factor alpha stimulated gene/inducible protein 6(TSG-6) and interleukin 6(IL-6).Results Among the three kinds of scald models,compared with the MODEL,the MEBO and the COL 1D/T can significantly shorten the wound healing time and significantly increase the number of wound microangiogenesis.At the same time,the test results showed that the MEBO and COL 1D/T can effectively reduce the content of inflammatory factors TNF-α,IL-6 and TSG-6 in the serum of scalded rats and significantly increase the content of wound factor of bFGF and EGF.Conclusion The COL 1D/T has significant anti-inflammatory and pro-healing effects on deep second-degree scald wounds compared with other long-term administration groups.The mechanism of collagenase acting on deep s
