摘 要:目的 探讨乌鲁木齐地区健康体检人群糖化血红蛋白(Glycated hemoglobin A1c, HbA1c)水平的分布并初步制定参考区间,验证其可靠性。方法 通过新疆医科大学附属中医医院实验室信息系统收集2014年1月至2023年10月的42321例健康体检人群的HbA1c检测结果,分析比较不同性别、不同年龄HbA1c水平的组间差异和相关性,利用非参数法建立HbA1c参考区间,并进行初步验证。结果 男女两组HbA1c水平差异有统计学意义(Z=-5.995 P<0.05),HbA1c检测结果与年龄呈现正相关性(r=0.390 P<0.05);男、女两组的HbA1c检测水平与相应年龄呈现正相关性(r=0.118 P<0.001;r=0.216 P<0.001)。HbA1c生物参考区间分别为男性4.9%-6.37%,女性4.86%-6.37%,年龄18-30岁、30-50岁、50-70岁及≥70岁的参考范围分别为4.70%-5.90%,4.85%-6.20%,5.0%-6.40%及5.10%-6.46%;男性18-30岁、30-50岁、50-70岁、≥70岁参考区间为4.79%-5.91%、4.90%-6.26%、5.0%-6.40%、5.09%-6.44%;女性18-30岁、30-50岁、50-70岁、≥70岁参考区间为4.70%-5.08%、4.80%-6.12%、5.00%-6.40%、5.15%-6.49%。结论 乌鲁木齐地区HbA1c水平在性别、年龄方面具有差异及相关性,初步建立了符合本实验室的参考区间,为临床应用提供了依据。ObjectiveTo explore the distribution of Glycated hemoglobin A1c(HbA1c) levels in the population of health checkups in Urumqi region and formulate a preliminary reference interval to verify its reliability.MethodsThe HbA1c test results of 42,321 cases of health checkups from January 2014 to October 2023 were collected through the laboratory information system of the Affiliated Hospital of Traditional Chinese Medicine of Xinjiang Medical University, and the intergroup differences and correlations of HbA1c levels of different genders and ages were analyzed and compared, and the HbA1c reference interval was established using the nonparametric method and preliminarily verified.ResultsThe differences in HbA1c levels between male and female groups were statistically significant(Z=-5.995 P<0.05), and the HbA1c test results showed a positive correlation with age(r=0.390 P<0.05);the HbA1c test levels of male and female groups showed a positive correlation with the corresponding age(r=0.118 P<0.001;r=0.216 P<0.001).The biological reference ranges for HbA1c were 4.9%-6.37% for men and 4.86%-6.37% for women, and the reference ranges for ages 18-30, 30-50, 50-70 and ≥70 were 4.70%-5.90%, 4.85%-6.20%, 5.0%-6.40% and 5.10%-6.46%, respectively;for men 18-30, 30-50, 50-70,and ≥70 years of age reference ranges were 4.79%-5.91%, 4.90%-6.26%, 5.0%-6.40%, and 5.09%-6.44%;for females, the reference ranges for 18-30, 30-50, 50-70, and ≥70 years of age were 4.70%-5.08%, 4.80%-6.12%, 5.00%-6.40%, and 5.15%-6.49%.ConclusionThe HbA1c levels in Urumqi region have differences and correlations in gender and age, and the reference intervals conforming to our laboratory have been initially established, which provide a basis for clinical application.
