摘　　要：目的 探讨乙酰半胱氨酸雾化吸入联合环磷酰胺治疗对特发性间质性肺炎患者血气指标及纤维化指标的影响。方法 选取2021年7月-2023年2月在本院呼吸科救治的102例特发性间质性肺炎患者为研究对象,随机数表分为观察组与对照组,各51例,对照组行环磷酰胺治疗,观察组在对照组治疗基础上加乙酰半胱氨酸雾化吸入。记录两组患者治疗前后血气指标[动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))],纤维化指标[血沉(ESR)、类风湿因子(RF)、抗链球菌溶血素O(ASO)、C反应蛋白(CRP)、抗环瓜氨酸肽抗体(CCP)],狼疮鉴别指标[抗核抗体(ANA)、抗双链DNA、抗sm抗体、抗中性粒细胞胞浆抗体(ANCA)]。结果 治疗后观察组患者PaO_(2)水平明显高于对照组,PaCO_(2)、ESR、RF、ASO、CRP、CCP、ANA、抗双链DNA抗体、抗sm抗体阳性率及ANCA水平明显低于对照组(P<0.05)。结论 乙酰半胱氨酸雾化吸入联合环磷酰胺用于特发性间质性肺炎治疗可改善患者通气状况,减缓肺纤维化进程,抑制自身免疫疾病活动,建议临床推广使用。Objective To investigate the effect of acetylcysteine nebulization inhalation combined with cyclophosphamide treatment on blood gas and fibrosis indicators in patients with idiopathic interstitial pneumonia.Method 102 patients with idiopathic interstitial pneumonia who were treated in the respiratory department of our hospital from July 2021 to February 2023 were selected as the study subjects.They were randomly divided into a observation group and a control group,with 51 patients in each group.The control group received cyclophosphamide treatment,while the observation group received acetylcysteine nebulization inhalation on top of the control group.Before and after treatment,blood gas indicators of both groups of patients were recorded,including arterial oxygen partial pressure(PaO_(2))and arterial carbon dioxide partial pressure(PaCO_(2))levels;fibrosis indicators including erythrocyte sedimentation rate(ESR),rheumatoid factor(RF),anti streptolysin O(ASO),C-reactive protein(CRP)levels,and anti cyclic citrulline peptide antibody(CCP)levels;differential indicators of lupus including antinuclear antibodies(ANA),anti double stranded DNA,anti sm antibodies and anti neutrophil cytoplasmic antibodies(ANCA).Results After treatment,the PaO_(2)levels in the observation group were significantly higher than those in the control group,and PaCO_(2),ESR,RF the positive rates of ASO,CRP,CCP,ANA,anti double stranded DNA antibodies,anti sm antibodies,and ANCA levels in the control group were significantly lower than those in the control group(P<0.05).Conclusion Acetylcysteine nebulization inhalation combined with cyclophosphamide for the treatment of idiopathic interstitial pneumonia can improve patient ventilation,slow down the progression of pulmonary fibrosis,and inhibit autoimmune disease activity.It is recommended to promote clinical use.
