摘　　要：目的分析血清25-羟维生素D_(3)[25(OH)D_(3)]、微小核糖核酸-25-3p(miR-25-3p)、壳多糖酶3样蛋白1(CHI3L1)水平与慢性乙型肝炎肝纤维化程度及肝功能的关系。方法选取2021年12月—2022年12月我院收治的106例慢性乙型肝炎患者,依据患者肝纤维化程度(采用Ishak评分系统)分为0~1级组(30例)、2~3级组(41例)、4~6级组(35例);依据患者肝功能(采用ChildPugh分级)分为A级组(26例)、B级组(42例)、C级组(38例)。比较不同肝脏纤维化程度各组25(OH)D_(3)、miR-25-3p、CHI3L1、脾脏硬度值及肝脏硬度值;比较不同肝功能各组25(OH)D_(3)、miR-25-3p、CHI3L1、肝脏纤维化程度分级、脾脏硬度值及肝脏硬度值。结果不同肝脏纤维化程度组miR-25-3p、CHI3L1、脾脏硬度值和肝脏硬度值比较,0~1级组<2~3级组<4~6级组(P<0.05),而25(OH)D_(3)比较为0~1级组>2~3级组>4~6级组(P<0.05);经过Pearson相关分析,肝脏纤维化程度分级与miR-25-3p、脾脏硬度值、肝脏硬度值呈正相关(P<0.05),肝脏纤维化程度分级与25(OH)D_(3)呈负相关(P<0.05);不同肝功能各组miR-25-3p、CHI3L1、肝脏纤维化程度分级、脾脏硬度值和肝脏硬度值比较,A级组<B级组<C级组(P<0.05),而25(OH)D_(3)比较为A级组>B级组>C级组(P<0.05);Logistic回归分析结果显示,miR-25-3p水平、肝脏纤维化程度分级、脾脏硬度值、肝脏硬度值是慢性乙型肝炎患者肝功能较差的独立危险因素(P<0.05),25(OH)D_(3)水平是慢性乙型肝炎患者肝功能较差的保护因素(P<0.05)。结论慢性乙型肝炎患者随着乙肝纤维化分级增大,肝功能变差,血清25(OH)D_(3)降低,miR-25-3p、CHI3L1升高;25(OH)D_(3)、miR-25-3p、肝脏纤维化程度分级、脾脏硬度值和肝脏硬度值与慢性型肝炎患者肝纤维化及肝功能具有相关性。Objective The relationship between serum 25-hydroxyvitamin D_(3)[25(OH)D_(3)],minimal ribonucleic acid-25-3p(miR-25-3p),and chitosaccharidase 3-like protein 1(CHI3L1)and the degree of liver fibrosis and liver function of chronic hepatitis B were analyzed.Methods 106 patients with chronic hepatitis B treated in our hospital from December 2021 to December 2022 were selected and divided into grade 0-1 group(30 cases),grade 2-3 group(41 cases)and grade 4-6 group(35 cases)according to the degree of liver fibrosis(Ishak scoring system).According to the liver function of the patients(using ChildPugh classification),they were divided into A group(26 cases),B group(42 cases)and C group(38 cases).The values of 25(OH)D_(3),miR-25-3p,CHI3L1,spleen hardness and liver hardness in different liver fibrosis groups were compared,and the values of 25(OH)D_(3),miR-25-3p,CHI3L1,liver fibrosis degree grade,spleen hardness and liver hardness were compared in different liver function groups.Results The values of miR-25-3p,CHI3L1,spleen hardness and liver hardness in different liver fibrosis degree groups were compared,grade 0-1 group<2-3 group<4-6 group(P<0.05),and 25(OH)D_(3)was compared,grade 0-1 group>2-3 group>4-6 group(P<0.05).According to Pearson correlation analysis,liver fibrosis degree grade was positively correlated with miR-25-3p,spleen hardness and liver hardness(P<0.05),while liver fibrosis degree grade was negatively correlated with 25(OH)D_(3).The comparison of miR-25-3p,CHI3L1,liver fibrosis degree grade,spleen hardness value and liver hardness value among different liver function groups showed that group A<group B<group C(P<0.05),and group A>group B>group C(P<0.05)compared with 25(OH)D_(3).Logistic regression analysis was performed,and the analysis results were as follows:miR-25-3p level,liver fibrosis grade,spleen hardness value and liver hardness value were independent risk factors for poor liver function in patients with chronic hepatitis B(P<0.05),and 25(OH)D_(3)level was protective factor for poor liver function in
