摘  要:为确定经皮穴位电刺激(TEAS)提高立位耐力的最佳穴位组合,10名志愿者分别进行对照(C)组、内关(NT)组、内关+合谷+足三里+三阴交(NHZST)组、假内关+假合谷+假足三里+假三阴交(FT)组的TEAS。每组刺激间隔一周进行,清洗效应。刺激前、后进行蹲立实验(SST),确定提高心血管调整能力的最佳穴组。使用该穴组对志愿者进行即时TEAS(TA)、连续7天TEAS(PTA),通过头高位(HUT)立位应激+下体负压(LBNP)实验,观察总耐受时间(DNP)与累积应激指数(CSI)、心脏生理与泵血功能指标。处方研究结果表明:与C组、FT组比较,NT组、NHZST组在HR、SBP、MBP方面优于前两组;NHZST组与NT组比较,在HR、SBP、MBP方面,NHZST组优于NT组;C组与FT组比较无差异。量效关系结果表明:TA组、PTA组的DNP与CSI、DBP、MBP、每搏量(SV)、外周阻力(TPR)优于C组(P<0.05),且PTA组优于TA组(P<0.05);TA组的HR优于PTA组、C组(P<0.05),且PTA组优于C组(P<0.05)。TEAS内关+足三里+合谷+三阴交穴组可以提高志愿者对SST应激适应能力,提高HUT+LBNP时的立位耐力,连续7天刺激可以增效。To identify the optimal acupoint combination for enhancing standing endurance through transcutaneous electrical acupoint stimulation(TEAS),ten volunteers were assigned to four groups:Control group(C),Neiguan Group(NT),Neiguan+Hegu+Zusanli+Sanyinjiao(NHZST)group,and Sham Neiguan+Hegu+Zusanli+Sanyinjiao(FT)group.TEAS was administered with a one-week interval between sessions to eliminate potential carryover effects.Before and after stimulation,a squat-stand test(SST)was performed to determine the optimal acupoint combination for enhancing cardiovascular regulation.Based on the results from SST,volunteers received either immediate TEAS(TA)or continuous TEAS for 7 days(PTA).The head-up tilt(HUT)test combined with lower body negative pressure(LBNP)was used to evaluate duration of negative pressure(DNP),cumulative stress index(CSI),and cardiovascular and pumping function parameters.The results showed that,compared to group C and group FT,both group NT and NHZST exhibited significant improvements in heart rate(HR),systolic blood pressure(SBP),and mean blood pressure(MBP).When comparing group NHZST to group NT,Group NHZST showed superior results in HR,SBP,and MBP.No significant differences were found between the C and FT groups.The dose-response analysis indicated that the DNP,CSI,diastolic blood pressure(DBP),MBP,stroke volume(SV),and total peripheral resistance(TPR)were significantly better in both the TA and PTA groups compared to the C group(P<0.05),with the PTA group performing better than the TA group(P<0.05).Additionally,HR was significantly lower in the TA group compared to both the PTA and C groups(P<0.05),with the PTA group also outperforming the C group(P<0.05).The TEAS combination of Neiguan+Zusanli+Hegu+Sanyinjiao effectively improved volunteers’stress adaptation in the SST,enhanced standing endurance during the HUT+LBNP test,and showed an additive effect with continuous 7-day stimulation.
