血清SFRP5、sortilin、CysC对妊娠期糖尿病患者不良妊娠结局的预测价值

摘　　要：目的:分析血清分泌型卷曲相关蛋白5(SFRP5)、分拣蛋白(sortilin)、胱抑素C(CysC)水平联合检测对妊娠期糖尿病患者妊娠结局的预测价值。方法:回顾性选取2022年4月—2024年2月某院137例妊娠期糖尿病患者为研究对象,根据患者妊娠结局分为妊娠良好组(n=98)和妊娠不良组(n=39)。收集患者入院时一般资料及实验室检测指标,采用多因素Logistic回归方程分析妊娠期糖尿病患者妊娠结局的影响因素,绘制受试者工作特征(ROC)曲线分析血清SFRP5、sortilin、CysC水平对患者妊娠结局的预测价值。结果:与妊娠良好组相比,妊娠不良组血清空腹血糖(FPG)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、sortilin、CysC水平升高,SFRP5水平降低,差异均有统计学意义(P<0.05);多因素Logistic回归分析发现血清FPG、TG、LDL-C、FINS、HOMA-IR、sortilin、CysC水平为妊娠不良的危险因素(P<0.05),血清SFRP5水平为妊娠不良的保护因素(P<0.05)。ROC曲线分析发现血清SFRP5、sortilin、CysC水平预测妊娠结局不良的AUC分别为0.785、0.835、0.803,三者联合预测妊娠结局不良的AUC为0.917。结论:妊娠期糖尿病患者血清SFRP5、sortilin、CysC水平异常与妊娠结局相关,三者联合检测可提高临床预测价值,为临床提供参考。Objective:To analyze the predictive value of combined detection of serum levels of secreted frizzled related protein 5(SFRP5),sortilin and cystatin C(CysC)for adverse pregnancy outcomes in patients with gestational diabetes mellitus.Methods:A total of 137 patients with gestational diabetes mellitus in a hospital from April 2022 to February 2024 were selected as research objects.They were divided into a good pregnancy outcome group(n=98)and an adverse pregnancy outcome group(n=39)according to the pregnancy outcomes during follow-up.Clinical data and laboratory indicators of the patients at admission were collected.Logistic regression was used to analyze the influencing factors of pregnancy outcomes of the diabetes patients during pregnancy.The ROC curve of the subjects was drawn to analyze the predictive value of serum SFRP5,sortilin,CysC levels on the pregnancy outcomes of the patients.Results:Compared with the group of good pregnancy,the levels of fasting plasma glucose(FPG),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),fasting insulin(FINS),insulin resistance index(HOMA-IR)as well as serum sortilin and CysC in the group of poor pregnancy were significantly increased,while the level of SFRP5 was significantly decreased(P<0.05).Logistic regression analysis revealed that serum levels of FPG,TG,LDL-C,FINS,HOMA-IR,sortilin,and CysC were risk factors for gestational abnormalities(P<0.05),while serum levels of SFRP5 were protective factors for gestational abnormalities(P<0.05).ROC curve analysis denoted that serum SFRP5,sortilin,and CysC levels predicted adverse pregnancy outcomes with AUC values of 0.785,0.835,and 0.803,respectively.The AUC of the combined prediction of adverse pregnancy outcomes by the three was 0.917.Conclusion:The abnormal levels of serum SFRP5,sortilin,and CysC in patients with gestational diabetes are associated with pregnancy outcomes,and combined detection of the three can improve clinical predictive value and provide clinical reference.
