摘　　要：研究聚焦于“互联网+慢性病居家管理”领域,旨在探索多源异构数据融合技术与AI大模型的创新应用。通过整合多源异构数据,并应用先进的数据融合与分析方法,挖掘数据价值,结合AI大模型生成动态健康画像,实现对慢性病患者的实时疾病风险评估与精准病情发展预测。研究成果将为慢性病居家管理提供智能化、个性化的解决方案,提高了管理效率与质量,改善患者的生活质量。The research focuses on the field of“Internet+chronic disease home management”,aiming to explore the innovative application of multi-source heterogeneous data fusion technology and AI big model.By integrating heterogeneous data from multiple sources and applying advanced data fusion and analysis methods,we can explore the value of data and generate dynamic health profiles using AI models,achieving real-time disease risk assessment and accurate disease development prediction for chronic disease patients.The research results will provide intelligent and personalized solutions for home management of chronic diseases,improve management efficiency and quality,and enhance patients̓quality of life.
