雷火灸联合低频脉冲电刺激对脑卒中后偏瘫患者肌张力、患肢功能和神经功能的影响

摘　　要：目的:探究雷火灸联合低频脉冲电刺激对脑卒中后偏瘫患者肌张力、患肢功能和神经功能的影响。方法:纳入本院2021年12月—2023年12月收治的120例脑卒中后偏瘫患者,采用随机数字表法将上述患者分为对照组(采用常规治疗+低频脉冲电刺激)与观察组(对照组基础上+雷火灸),各60例,比较两组肌张力、患肢功能、神经功能、脑血流动力学与生活质量。结果:治疗后,观察组0级和1级肌张力分级占比高于对照组,差异具有统计学意义(P<0.05);治疗后,观察组患侧上下肢功能评分均高于对照组,差异具有统计学意义(P<0.05);治疗后,观察组神经功能评分低于对照组,差异具有统计学意义(P<0.05);治疗后,观察组搏动指数(PI)、双侧大脑中动脉的阻力指数(RI)均低于对照组,平均血流速度(Vm)高于对照组,差异具有统计学意义(P<0.05);与治疗前比较,治疗后,两组生活质量评分均明显升高(P<0.05),且观察组生活质量评分高于对照组,差异具有统计学意义(P<0.05)。结论:对于脑卒中后偏瘫患者而言,采用雷火灸联合低频脉冲电刺激可改善肌张力、神经功能及脑血流动力学,增强患肢功能,提升生活质量,具有应用价值。Objective:To explore the effect of thunder-fire moxibustion combined with low-frequency pulse electrical stimulation on muscle tension,affected limb function and neurological function in patients with post-stroke hemiplegia.Methods:120 patients with post-stroke hemiplegia were randomly divided into the control group(n=60) and the observation group(n=60).The control group was treated with routine treatment plus low-frequency pulse electrical stimulation;on which basis,the observation group was also treated with thunder-fire moxibustion.Muscle tension,affected limb function,neurological function,cerebral hemodynamics and quality of life were compared between the two groups.Results:After the treatment,the proportions of grade 0 and grade 1 muscle tension were higher in the observation group than those in the control group(P<0.05).The scores of upper and lower limb function of the affected side were higher in the observation group than those in the control group after the treatment(P<0.05).The neurological function score was lower in the observation group than that in the control group after the treatment(P<0.05).The pulsatility index(PI) and resistance index(RI) of bilateral middle cerebral arteries were lower,whereas the mean blood flow velocity(Vm) was higher in the observation group than those in the control group after the treatment(P<0.05).The score of quality of life was higher after the treatment in both groups(P<0.05),and the score in the observation group was higher than that in the control group after the treatment(P<0.05).Conclusion:For patients with post-stroke hemiplegia,thunder-fire moxibustion combined with low-frequency pulse electrical stimulation can improve muscle tension,neurological function and cerebral hemodynamics,enhance affected limb function and promote quality of life.The therapy has a high application value.
