摘 要:目的:观察头穴透刺对脑出血大鼠神经行为学和继发性脑水肿的影响,并探讨其干预TWEAK/Fn14/STAT3信号通路促进血管新生的相关机制。方法:120只SD大鼠随机分为空白组、假手术组、模型组、针刺组和针刺+Stattic组,共5组,每组24只,以大鼠尾状核自体动脉血注入法复制脑出血模型,评定造模成功后,针刺组、针刺+Stattic组给予“百会”透刺“曲鬓”穴电针干预治疗;正常组、假手术组和模型组不作干预。在开始治疗后第6 h、1天、3天和7天分别对各组大鼠进行Bederson评分、Rosenberg评分及脑含水量测定;Western blot及Real-time PCR检测大鼠血肿侧脑皮质TWEAK、Fn14及STAT3蛋白表达和mRNA表达,ELISA检测大鼠血肿侧脑皮质MMP-2、MMP-9含量。结果:模型组、针刺组和针刺+Stattic组在不同时间点的Bederson评分、Rosenberg评分和脑含水量均较空白组和假手术组显著增加,差异具有统计学意义(P<0.05);与模型组比较,针刺组的上述评分和脑含水量在干预后第1天、3天和7天均可见显著降低,差异具有统计学意义(P<0.05),而针刺+Stattic组在不同时间点的上述评分和脑含水量,差异均无统计学意义(P>0.05);与针刺组比较,针刺+Stattic组的Bederson评分、Rosenberg评分和脑含水量在干预后第1天、3天和7天均可见显著升高,差异具有统计学意义(P<0.05)。干预后,针刺组大鼠的TWEAK、Fn14及STAT3蛋白和mRNA表达均较模型组和针刺+Stattic组显著增加,差异具有统计学意义(P<0.05),MMP-2、MMP-9含量均较模型组和针刺+Stattic组显著降低,差异具有统计学意义(P<0.05);模型组和针刺+Stattic组间上述指标比较,差异均无统计学意义(P>0.05)。结论:头穴透刺干预脑出血大鼠能够明显改善大鼠的神经行为学和继发性脑水肿,该作用机制可能是通过调控TWEAK/Fn14/STAT3信号通路促进血管新生实现的。Objective:To observe the effect of scalp point through point acupuncture on neuroethology and secondary cerebral edema in rats with intracerebral hemorrhage,and to explore the mechanism of its intervention in TWEAK/Fn14/STAT3 signaling pathway to promote angiogenesis.Methods:A total of 120 SD rats were randomly divided into the blank group,the sham operation group,the model group,the acupuncture group and the acupuncture+Stattic group,with 24 rats in each group.The intracerebral hemorrhage model was simulated by caudate nucleus autogenous arterial blood injection in rats.After the successful modeling,the acupuncture group and the acupuncture+Stattic group were treated with electro-needling Baihui(DU20) through Qubin(GB7),whereas the blank group,the sham operation group and the model group were not given any intervention.At 6 h,1 d,3 d and 7 d after the treatment,the Bederson score,Rosenberg score and brain water content were determined;the protein and mRNA expressions of TWEAK,Fn14 and STAT3 were detected by Western blot and Real-time PCR;the contents of MMP-2 and MMP-9 in hemotoma-side cortex were detected by ELISA.Results:The Bederson score,Rosenberg score and brain water content were significantly increased in the model group,the acupuncture group and the acupuncture+Stattic group compared with those in the blank group and the sham operation group at different time points(P<0.05);compared with those in the model group,the Bederson score,Rosenberg score and brain water content were significantly decreased in the acupuncture group on 1 d,3 d and 7 d after the intervention(P<0.05),in which there were no significant differences in the acupuncture+Stattic group at different time points(P>0.05).Compared with those in the acupuncture group,the Bederson score,Rosenberg score and brain water content were significantly increased in the acupuncture+Stattic group on 1 d,3 d and 7 d after the intervention(P<0.05).After the intervention,the protein and mRNA expressions of TWEAK,Fn14 and STAT3 were significantly increased in
