基于“阳气来复”理论探讨温针灸联合温脉通络丸治疗下肢动脉硬化闭塞症的临床研究

摘　　要：目的:基于“阳气来复”理论探讨温针灸与温脉通络丸联合应用治疗下肢动脉硬化闭塞症的临床疗效。方法:选取河南省中医院2023年4月—2024年4月间收住院的下肢动脉硬化闭塞症(寒湿阻络证)患者90例,随机分为对照组和治疗组,每组45例。两组患者给予基础治疗,对照组加服温脉通络丸,治疗组在对照组的基础上联合温针灸。比较1个月后各组的中医症候积分、ABI、足背动脉血管内径、IL-6、TNF-α以及Hcy变化情况。结果:治疗1个月后,与对照组比较,治疗组在中医症候积分、IL-6、TNF-α与Hcy方面显著降低,差异具有统计学意义(P<0.05);ABI、足背动脉血管内径值显著升高,差异具有统计学意义(P<0.05);治疗组总有效率为88.89%(40/45),显著超过对照组的71.12%(32/45),差异具有统计学意义(P<0.05)。结论:温针灸与温脉通络丸联合应用治疗下肢动脉硬化闭塞症具有明确的临床疗效,可明显改善下肢发凉、麻木和疼痛等症状以及下肢缺血和血流动力学情况,并有效抑制炎症因子,提升患者生活质量。Objective:To explore the clinical efficacy of needle-warmming moxibustion combined with Wenmai Tongluo Pill in treating arteriosclerosis obliterans(ASO)of lower limbs based on Yang-Qi restoration theory.Methods:A total of 90 patients with lower limb ASO(cold-dampness obstruction syndrome)were randomly divided into the control group and the treatment group,with 45 patients in each group.The control group was treated with basic treatment combined with Wenmai Tongluo Pill;on which basis,the treatment group was also treated with needle-warming moxibustion.The changes in TCM symptoms scores,ABI,dorsal foot artery vessel diameter,IL-6,TNF-α and Hcy were compared between the two groups after one month of the treatment.Results:After one month of the treatment,the scores of TCM symptoms scores,the levels IL-6,TNF-α,and Hcy significantly decreased(P<0.05),and ABI and the diameter of the dorsal foot artery significantly increased in the treatment group compared with those in the control group(P<0.05).The total effective rate of the treatment group was 88.89%(40/45),which was significantly higher than 71.12%(32/45)of the control group(P<0.05).Conclusion:Needle-warming acupuncture combined with Wenmai Tongluo Pill has a definite clinical effect in treatment of ASO of lower limbs,which can significantly improve the symptoms of lower limb coldness,numbness and pain,as well as lower limb ischemia and hemodynamics,effectively inhibit inflammatory factors,and improve life quality of the patients.
