基于HPA轴探讨针刺督脉腧穴治疗肠易激综合征的作用机制

摘　　要：现代医学认为肠易激综合征的发病与下丘脑-垂体-肾上腺轴(HPA轴)的功能亢进有关。中医学认为肠易激综合征的病位在肠,与肝、脾关系密切,脑神失职则是IBS-D发病的关键,而督脉“入络于脑”又与胃肠脏腑经络联系密切,能够实现神与脾胃同调。因此,本研究简述督脉腧穴治疗IBS-D的理论基础及针法,简要分析HPA轴与IBS-D的关系,并探讨针刺督脉腧穴调控HPA轴治疗IBS-D的作用机制,指明针刺督脉腧穴治疗IBS-D的可行性。Modern medicine believes that irritable bowel syndrome(IBS)is associated with the hyperfunction of the hypothalamic-pituitary-adrenal axis(HPA axis).Traditional Chinese medicine believes that the disease location of IBS lies in the Intestines,which is closely associated with the Liver and Spleen,and brain-spirit dysfunction is the key to the pathogenesis of IBS-D.Du meridian enters collateral through brain and is closely related to the gastrointestinal organs and meridians,which can achieve the harmony between the spirit and the Spleen and Stomach.This study briefly described the theoretical basis and acupuncture manipulations of the treatment of IBS-D,briefly analyzed the correlation between the HPA axis and IBS-D,and discussed the action mechanism of needling acupoints of Du meridian to regulate the HPA axis in the treatment of IBS-D.It pointed out the feasibility of needling acupoints of Du meridian to treat IBS-D.
