腔内射频联合点式剥脱及硬化剂治疗下肢静脉曲张的疗效观察

摘　　要：目的评估采用腔内射频联合点式剥脱、硬化剂治疗下肢静脉曲张的手术效果。方法回顾性收集2022年7月至2023年11月昆山市中医院采用射频消融闭合大隐静脉主干,点式剥脱联合硬化剂处理小腿曲张静脉的81例下肢静脉曲张患者临床资料,观察患者术后并发症情况以及静脉临床严重程度评分(venous clinical severity score,VCSS)、慢性静脉功能不全生活质量问卷(chronic venous insufficiency questionnaire,CIVIQ)评分的变化,评估手术效果及安全性。结果患者手术均取得成功,早期并发症主要包括下肢水肿、瘀斑、皮肤瘙痒等,多在术后6个月内缓解,无深静脉血栓形成、肺栓塞等严重并发症发生。术后12个月大隐静脉闭合率100%。患者术后第3、6、12个月随访,各时间点的VCSS和CIVIQ评分较之前均有明显改善,差异有统计学意义(P<0.05)。结论采用腔内射频联合点式剥脱、硬化剂治疗下肢静脉曲张的手术方式临床效果满意,术后并发症少,患者临床症状和生活质量较术前明显改善。Objective To evaluate the surgical outcomes of endovenous radiofrequency ablation combined with stripping and sclerotherapy on varicose veins of lower extremities.Methods The clinical data of 81 patients with varicose veins of lower extremities who underwent radiofrequency ablation for closure of the great saphenous veins,combined with stripping and sclerotherapy for calf varicose veins at Kunshan Hospital of Traditional Chinese Medicine between July 2022 and November 2023 were collected retrospectively.Postoperative complications,venous clinical severity scores(VCSS)and chronic venous insufficiency questionnaire(CIVIQ)scores were observed to assess surgical efficacy and safety.Results All procedures were successful.Early complications included lower limb edema,ecchymosis and pruritus,which mostly resolved within 6 months postoperatively.No severe complications such as deep vein thrombosis or pulmonary embolism occurred.The closure rate of the great saphenous vein was 100%at 12 months postoperatively.Follow-ups at 3,6,12 months postoperatively showed significant improvements in VCSS and CIVIQ scores compared to preoperative values,with statistically significant differences(P<0.05).Conclusion Endovenous radiofrequency ablation combined with stripping and sclerotherapy on varicose veins of lower extremities has satisfactory clinical outcomes,minimal postoperative complications,and significant improvements in patients’symptoms and quality of life.
