摘 要:目的研究探讨IQon光谱CT(IQon-spectral CT)技术在临床肺癌诊断中的应用价值。方法回顾性选取2021年5月至2023年5月于我院经病理学确诊的105例肺癌患者为研究对象,所有患者均接受IQon光谱CT动脉期及静脉期增强扫描,并以病理学为“金标准”判断IQon光谱CT技术在肺癌中的检出情况,经肺癌切除及淋巴结清扫术后共获得淋巴结188枚,其中转移淋巴结有114个、非转移淋巴结74个,比较转移淋巴结与非转移淋巴结光谱参数[碘浓度(IC)、标准化IC(NIC)、能谱曲线斜率(λ_(H))、有效原子序数(Eff-Z)]之间的差异。结果105例肺癌病理检出情况包括小细胞癌39例、大细胞癌31例、腺癌25例、鳞癌10例,IQon光谱CT技术检出情况包括小细胞癌37例、大细胞癌28例、腺癌23例、鳞癌9例、良性病变8例,诊断符合率分别为94.87%、90.32%、92.00%、90.00%;转移淋巴结与非转移淋巴结光谱参数相比,转移淋巴结组动脉期及静脉期IC、NIC、Eff-Z均小于非转移淋巴结组(P<0.05),而λ_(H)大于非转移淋巴结组(P<0.05)。结论IQon光谱CT技术在肺癌及判断疾病类型上均有着较高的诊断价值。另外其在评估肺癌淋巴结转移也能发挥重要作用,能借助IC、NIC、λ_(H)、Eff-Z等光谱参数的比较为肺癌术前淋巴结鉴别提供可靠信息。Objective To investigate the application value of IQon spectral CT in clinical diagnosis of lung cancer.Methods A retrospective study was conducted among 105 patients with lung cancer diagnosed by pathology in the hospital from May 2021 to May 2023.All patients enrolled received enhanced IQon spectral CT scan in arterial phase and venous phase.With pathology as the gold standard,the value of IQon spectral CT in the diagnosis of lung cancer was evaluated.A total of 188 lymph nodes were obtained after resection of lung cancer and lymph node dissection,including 114 metastatic lymph nodes and 74 non-metastatic lymph nodes.Spectral parameters[iodine concentration(IC),normalized IC(NIC),slope of energy spectrum curve(λ_(H))and effective atomic number(Eff-Z)]of metastatic lymph nodes and non-metastatic lymph nodes were comparatively analyzed.Results Pathological results showed that among the 105 cases of patients with lung cancer,there were 39 cases of small cell cancer,31 cases of large cell cancer,25 cases of adenocarcinoma,and 10 cases of squamous cell carcinoma.IQon spectral CT detected 37 cases of small cell cancer,28 cases of large cell cancer,23 cases of adenocarcinoma,9 cases of squamous cell carcinoma,and 8 cases of benign lesions.The diagnostic accordance rates were 94.87%,90.32%,92.00%and 90.00%.IC,NIC,and Eff-Z in arterial phase and venous phase of the metastatic lymph node group were lower than those in the nonmetastatic lymph node group(P<0.05),andλ_(H)was greater than that in the non-metastatic lymph node group(P<0.05).Conclusion IQon spectral CT is of high value in diagnosing lung cancer and determining the type of disease.In addition,it plays an important role in evaluating lymph node metastasis in lung cancer.Comparison of spectral parameters such as IC,NIC,λ_(H)and cEff-Z can provide reliable information for differentiation of lymph nodes before lung cancer surgery.
