摘 要:目的探析薄层CT在肺亚实性结节(SSN)动态随访中的价值。方法回顾性分析本院2020年10月至2023年10月行薄层CT检查的SSN患者的临床资料,根据其是否有实性成分分为部分实性结节(PSN)组和磨玻璃结节(GGN)组,比较两组首次薄层CT影像学表现及动态随访期结节影像学特征的差异,评价CT在SSN动态随访中的应用价值。结果研究共纳入297例患者,其中99例为PSN,198例为GGN。PSN组与GGN组的结节数目、结节长径、结节CT值、CT比值以及6~12月、12~24月复查薄层CT的频率组间比较差异有统计学意义(P<0.05)。薄层CT在不同时间段内对不同直径PSN的动态变化检出率比较差异均有统计学意义(P<0.05),且薄层CT在0~3月、3~6月对直径>8.0 mm PSN动态变化的检出率明显高于直径3.0~8.0 mm的PSN(P<0.05)。薄层CT在不同时间段内对直径3.0~5.0 mm GGN动态变化的检出率差异无统计学意义(P>0.05),对直径>5.0 mm的GGN在0~3月和3~6月动态变化的检出率比较差异有统计学意义(P<0.05)。在PSN与GGN动态随访变化比较上,薄层CT在0~3月、3~6月及12~24月对PSN的动态变化检出率明显高于GGN组(P<0.05)。结论SSN的薄层CT参数在不同结节类型中存在明显差异,PSN与GGN随访不同阶段表现各异,使用薄层CT扫描并行动态观察,对于疾病的早期诊断及治疗策略的选择有重要参考价值。Objective To explore the value of thin-layer CT in dynamic follow-up of pulmonary subsolid nodule(SSN).Methods A retrospective analysis was performed on the clinical data of patients with SS N who underwent thin-layer CT examination in the hospital from October 2020 to October 2023.Patients included were divided into partial solid nodule(PSN)group and ground-glass nodule(GGN)group according to the the presence or absence of solid component.The first thin-layer CT imaging findings and imaging features of nodules during dynamic follow-up period were compared between the two groups.The application value of CT in dynamic follow-up of SSN was evaluated.Results A total of 297 patients were included in this study,including 99 patients with PSN and 198 patients with GGN.There were statistically significant differences between PSN group and GGN group in terms of the number of nodules,long diameter of nodule,CT value of nodule,CT ratio,and thin-layer CT reexamination frequencies during 6-12 months and 12-24 months(P<0.05).The percentages of dynamic changes in PSN of 3.0-8.0 mm and>8.0 mm in diameter during different follow-up periods showed statistically significant differences(P<0.05).The percentages of dynamic changes in PSN>8.0 mm in diameter during 0-3 months and 3-6 months were significantly higher compared to PSN of 3.0-8.0 mm in diameter(P<0.05).The percentages of dynamic changes in GGN of 3.0-5.0 mm in diameter during different follow-up time periods showed no statistically significant difference(P>0.05).The percentages of dynamic changes in GGN>5.0 mm in diameter during 0-3 months and 3-6 months showed statistically significant differences(P<0.05),but there was no statistically significant difference during the other time periods(P>0.05).Conclusion There are significant differences in thin-layer CT parameters among different types of nodules.The manifestations of PSN and GGN varies at different stages of follow-up.Using thin-layer CT scan for dynamic observation can provide important reference for early diagnosis
