摘　　要：目的对于腰椎间盘突出和髋部疾病并存的病例,常常因复杂而非典型的临床表现导致误诊和漏诊,通过对1例腰椎间盘突出症(LDH)合并髋关节一过性骨质疏松(TOH)患者的诊疗过程进行汇报和相关文献分析,以总结相关诊治经验。方法对收治的1例TOH患者的诊疗过程进行报告,同时结合相关文献对该疾病的病因、诊断和治疗进行复习。结果两种临床疾病的症状存在很大共性,尽管患者在经过治疗后症状得到一定程度改善,但并未完全消失,考虑两种疾病与患者的临床表现均具有相关性,并且在严重程度方面可能存在叠加效应。结论笔者观察到当TOH与LDH同时存在时可能放大了疾病对患者的损害,细致的查体与客观的影像学资料十分重要,临床诊疗过程中不应过分强调影像学检查而忽视查体的重要性,避免造成误诊或漏诊。Objective Complex and atypical clinical manifestations of lumbar disc herniation combined with hip disease often lead to misdiagnosis and missed diagnosis.In this paper,the diagnosis and treatment process of a patient with lumbar disc herniation(LDH)complicated with transient osteoporosis of the hip joint was reported and the relevant literature was analyzed to summarize the relevant diagnosis and treatment experience.Methods The diagnosis and treatment process of a patient with TOH was reported.The etiology,diagnosis,and treatment of the disease were reviewed in combination with relevant literature.Results Symptoms of the two clinical diseases shared great similarities.Although the symptoms of the two clinical diseases improved to a certain extent after treatment,they did not disappear completely.The authors considered that the two diseases were related to the clinical manifestations of the patients,and there may be a superimposed effect in terms of severity.Conclusion The author observes that when TOH and LDH exist at the same time,the disease′s damage to patients may be magnified,and careful physical examination and objective imaging data are very important.The importance of imaging examination should not be overemphasized and the physical examination should not be ignored in the process of clinical diagnosis and treatment.Otherwise,missed diagnosis or misdiagnosis may occur.
