程控硬膜外间歇脉冲注入法在初产妇分娩镇痛效果及对新生儿阿普加评分的影响  

The Effect of Programmed Epidural Intermittent Pulse Injection on Labor Analgesia in Primiparous Women and Its Impact on Neonatal Apgar Scores

摘　　要：目的研究初产妇分娩中应用程控硬膜外间歇脉冲注入法(PIEB)对镇痛效果、新生儿阿普加(Apgar)评分的影响。方法选择本院100例初产妇为研究对象,入组时间均为2022年1月至2024年6月,按照数字随机法分为两组,对照组采用持续硬膜外泵注镇痛,实验组采用PIEB镇痛,对比两组镇痛效果、新生儿Apgar评分。结果两组镇痛前、给药1h时VAS评分、第一产程、第二产程时间及新生儿出生1、5、10min后Apgar评分、PO_(2)、PCO_(2)水平无差异(P>0.05),实验组给药2h VAS(2.23±0.98)分、宫口全开(3.51±1.03)分、胎儿娩出时(3.94±1.09)分、总麻醉药液剂量(62.83±7.29)mL低于对照组(P<0.05)。实验组产妇镇痛满意度(4.62±0.25)分高于对照组(P<0.05),实验组产妇运动阻滞率(4.00%vs 16.00%)低于对照组(P<0.05)。结论初产妇分娩中实施PIEB镇痛的效果理想,能降低运动阻滞率,且对新生儿无明显不良影响。Objective To study the effect of programmed intermittent epidural injection(PIEB)on the analgesic effect and neonatal Apgar score in primiparous women during delivery.Methods 100 primiparous women in our hospital were selected as the research subjects.The enrollment period was from January 2022 to June 2024.They were divided into two groups according to the numerical random method.The control group received continuous epidural pump analgesia,and the experimental group PIEB was used for analgesia,and the analgesic effects and neonatal Apgar scores were compared between the two groups.Results There were no differences in VAS scores,first stage of labor,second stage of labor time,Apgar score,PO_(2),and PCO_(2)levels between the two groups before analgesia,1 hour after administration,and 1,5,and 10 minutes after birth(P>0.05),in the experimental group,the 2-h VAS(2.23±0.98)minutes,the full cervix(3.51±1.03)minutes,the fetal delivery(3.94±1.09)minutes,and the total anesthetic liquid dose(62.83±7.29)mL were lower than Control group(P<0.05).The maternal analgesia satisfaction score in the experimental group(4.62±0.25)was higher than that in the control group(P<0.05),and the maternal motor block rate in the experimental group(4.00%vs 16.00%)was lower than that in the control group(P<0.05).Conclusion The effect of PIEB analgesia in primiparous women during delivery is ideal,it can reduce the rate of motor block and has no obvious adverse effects on newborns.
