皮瓣转移联合改良Brisson术用于小儿隐匿性阴茎治疗中的效果研究

摘　　要：目的探讨分析小儿隐匿性阴茎治疗中皮瓣转移联合改良Brisson术的应用效果。方法选取2022年1月至2024年9月我院收治的110例隐匿性阴茎患儿,以数字表法随机分为2组,对照组(皮瓣转移联合改良Devine术)和观察组(皮瓣转移联合改良Brisson术)各55例,对比分析两组治疗情况。结果观察组术中出血量较对照组少,术后阴茎长度较对照组长(P<0.05)。术后24h,两组血清皮质醇(Cor)、肾上腺素(E)水平无明显差异(P>0.05)。两组并发症相比,观察组更少(P<0.05)。两组满意度相比,观察组更高(P<0.05)。结论小儿隐匿性阴茎治疗中,皮瓣转移联合改良Brisson术或联合改良Devine术治疗均安全有效,但皮瓣转移联合改良Brisson术术中出血量更少,术后阴茎长度延伸效果更好,且并发症更少,家属满意度更高。Objective To investigate the application effect of flap transfer combined with improved Brisson in the treatment of concealed penis in children.Methods A total of 110 children with concealed penis admitted to our hospital from January 2022 to September 2024 were randomly divided into 2 groups by digital table method,55 cases in each group(control group(flap transfer combined with improved Devine surgery)and observation group(flap transfer combined with improved Brisson surgery).The treatment of the two groups was compared and analyzed.Results The amount of blood loss in the observation group was less than that in the control group,and the length of the penis after operation was longer than that in the control group(P<0.05).There were no significant differences in serum cortisol(Cor)and epinephrine(E)levels between the two groups 24h after operation(P>0.05).Compared with the two groups,the complications in the observation group were less(P<0.05).Compared with the two groups,the satisfaction of observation group was higher(P<0.05).Conclusion In the treatment of pediatric occult penis,flap transfer combined with modified Brisson surgery or modified Devine surgery is safe and effective,but flap transfer combined with modified Brisson surgery has less blood loss,better postoperative penile length extension,fewer complications,and higher family satisfaction.
