摘　　要：目的探究血浆置换与血液滤过联合治疗川崎病休克综合征患儿的效果及对容量相关指标、炎症水平的影响。方法选取我院2020年1月至2023年1月期间收治的31例川崎病休克综合征患儿,以抽签法分组。对照组15例给予连续性静脉-静脉血液滤过(CVVH)治疗,观察组16例增加血浆置换治疗,对比炎症因子水平:白细胞介素(IL)-1β、IL-6、肿瘤坏死因子-ɑ(TNF-ɑ)水平;容量相关指标:每搏输出变异(SVV)、胸腔液体水平(TFC)、心输出量(CO)、外周血管阻力(SVR)以及短期预后。结果治疗后观察组IL-1β、IL-6、TNF-ɑ、SVV、TFC、SVR水平低于对照组,28d生存率、CO水平高于对照组,有统计学意义(P<0.05)。结论血浆置换联合血液滤过对川崎病休克综合征患儿治疗,可有效降低炎症反应,维持血流动力学稳定状态,提升生存率。Objective To explore the effect of plasmapheresis and hemofiltration in the treatment of Kawasaki disease shock syndrome in children and its influence on volumetric indicators and inflammation levels.Methods 31 children with Kawasaki disease shock syndrome admitted to our hospital from January 2020 to January 2023 were divided into groups by drawing lots.Continuous veno-venous hemofiltration(CVVH)was given to 15 patients in the control group,and plasma exchange was added to 16 patients in the observation group.The levels of inflammatory factors including interleukin-1β,IL-6 and TNF-alpha were compared.Volumetric indicators:Stroke output variation(SVV),pleural fluid level(TFC),cardiac output(CO),peripheral vascular resistance(SVR),and short-term prognosis.Results After treatment,the levels of IL-1β,IL-6,TNF-alpha,SVV,TFC and SVR in the observation group were lower than those in the control group,and 28d survival rate and the level of CO was higher than that in the control group,with statistical significance(P<0.05).Conclusion Plasma exchange combined with hemofiltration can effectively reduce inflammation,maintain hemodynamic stability and improve survival rate in Kawasaki shock syndrome children.
