摘　　要：目的分析影响耳内镜下手术治疗耳硬化症疗效的因素。方法回顾性分析2020年1月至2024年6月在本院接受耳内镜手术治疗的218例耳硬化症患者的临床资料,包括年龄、身体体质量指数(BMI)、病程、性别、患耳侧情况等,根据术后疗效将患者分为无效组与有效组。通过单因素分析、多因素Logistic回归分析探讨影响耳硬化症术后疗效的因素。结果218例患者中,有36例(16.51%)治疗无效,182例(83.49%)患者术后听力恢复正常。单因素分析结果显示,两组的年龄、病程、术前气导(AC)平均阈值、术前气骨导差(ABG)比较,差异具有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,高龄、术前高AC平均阈值、术前高ABG是影响患者术后疗效的危险因素(P<0.05)。结论高龄、术前高AC平均阈值和术前高ABG是影响耳内镜下手术治疗耳硬化症疗效的重要因素。Objective To analyze the factors affecting the efficacy of endoscopic ear surgery in the treatment of otosclerosis.Methods The clinical data of 218 patients with otosclerosis who underwent endoscopic ear surgery in our hospital from January 2020 to June 2024 were retrospectively analyzed,including age,body mass index(BMI),course of disease,gender,affected ear side,etc.According to the postoperative efficacy,the patients were divided into ineffective group and effective group.Univariate and multivariate Logistic regression analysis were used to explore the factors affecting the postoperative efficacy of otosclerosis.Results Of the 218 patients,36 cases(16.51%)had ineffective treatment,and 182 cases(83.49%)had normal hearing after operation.Univariate analysis showed that there were significant differences in age,course of disease,average threshold of preoperative air conduction(AC)and preoperative air bone gap(ABG)between the two groups(P<0.05).Multivariate Logistic regression analysis showed that advanced age,preoperative high AC average threshold,and preoperative high ABG were risk factors affecting postoperative efficacy(P<0.05).Conclusion Advanced age,preoperative high AC average threshold and high ABG are important factors affecting the efficacy of endoscopic ear surgery for otosclerosis.
