摘　　要：目的深入分析本院门诊部中药饮片的合理用药情况,旨在为临床用药的安全性、合理性、有效性和经济性提供参考依据。方法回顾性选取2024年1月至6月本院门诊每月第一周的中药饮片处方记录,并随机抽取800张信息完整的处方样本,对处方中不规范情况、用药味数、单味药物超量使用情况以及人均处方消费金额、人均剂数情况进行统计分析。结果800张中药饮片处方中,共234张处方不规范,占29.250%,其中,临床诊断记录缺失或不完整有87张,占10.875%,煎煮剂量不合理有7张,占0.875%,特殊煎煮方法未标注或标注错误有88张,占11.000%,门诊剂量超过7日用量有52张,占6.500%。处方用药味数在11~18味之间的处方占大多数,共635张,达79.375%。单味药物超出药典用量的处方总共有175张,占21.875%;主要有30种药物超量使用,法半夏超量使用频率最高,超量处方占4.250%。在抽查的800张处方中,儿童231例,占28.875%,人均消费为91.892元;妇女患者536例,占67.000%,人均消费为161.491元。结论本院门诊中药饮片处方存在一定的不合理用药现象,亟需加强对中药饮片处方的规范化管理,需提高医务人员的专业素养,以确保临床用药的安全性和有效性。Objective To deeply analyze the rational use of traditional Chinese medicine decoction pieces in the outpatient department of our hospital,so as to provide reference for the safety,rationality,effectiveness and economy of clinical medication.Methods The prescription records of traditional Chinese medicine decoction pieces in the first week of each month in the outpatient department of our hospital from January to June 2024 were retrospectively selected,and 800 prescription samples with complete information were randomly selected.The non-standard situation in the prescription,the number of medications,the excessive use of single-dose drugs,the per capita prescription consumption amount,and the per capita number of doses were statistically analyzed.Results Among the 800 prescriptions of traditional Chinese medicine decoction pieces,234 prescriptions were not standardized,accounting for 29.250%,Among them,87 prescriptions were missing or incomplete clinical diagnosis records,accounting for 10.875%;there were 7 unreasonable decoction doses,accounting for 0.875%;there were 88 special decoction methods that were not marked or wrongly marked,accounting for 11.000%;there were 52 outpatient doses exceeding 7 days,accounting for 6.500%.Prescriptions with 11-18 flavors accounted for the majority,a total of 635,accounting for 79.375%.There were 175 prescriptions with single drug exceeding the dosage of pharmacopoeia,accounting for 21.875%;there were mainly 30 kinds of drugs overused,and the overuse frequency of Pinelliae Rhizoma was the highest,and the overdose prescription accounted for 4.250%.Among the 800 prescriptions,231 cases were children,accounting for 28.875%,and the per capita consumption was 91.892 yuan;there were 536 female patients,accounting for 67.000%,and the per capita consumption was 161.491 yuan.Conclusion There are some irrational drug use in the outpatient prescriptions of traditional Chinese medicine decoction pieces in our hospital.It is urgent to strengthen the standardized management of the prescript
