肱骨近端锁定钢板与解剖钢板内固定治疗老年肱骨近端骨折的效果对比分析

摘　　要：目的比较肱骨近端锁定钢板与解剖钢板内固定对老年肱骨近端骨折(PHF)的疗效。方法将于2022年1月至2024年2月在开封市人民医院骨科接受手术治疗的76例老年PHF患者,随机分为对照组(解剖钢板)与研究组(锁定钢板),各38例,比较两组手术与预后情况、肩关节功能、活动度及并发症。结果两组术中出血量与手术时间比较,差异无统计学意义(P>0.05);研究组住院与骨折愈合时间短于对照组,差异有统计学意义(P<0.05)。治疗前,两组Constant-Murley肩关节功能评分(CMS)各维度评分比较,差异无统计学意义(P>0.05);治疗后,研究组疼痛、日常生活活动、活动范围、三角肌力量等方面的功能评分均高于对照组,差异有统计学意义(P<0.05)。治疗前,两组肩关节活动度比较,差异无统计学意义(P>0.05);治疗后,研究组前屈、外展、内旋、外旋、上举角度均高于对照组,差异有统计学意义(P<0.05)。研究组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论与传统解剖钢板相比,锁定钢板内固定术后患者住院及骨折愈合时间更短,且肩关节功能改善情况更显著,肩关节活动度更高,还可降低并发症发生率。Objective To compare the efficacy of proximal humerus locking plates and anatomical plates in the internal fixation treatment of proximal humeral fractures(PHF)in the elderly.Methods A total of 76 elderly patients with PHF who underwent surgical treatment in the Department of Orthopedics at Kaifeng People's Hospital between January 2022 and February 2024 were randomly divided into a control group(anatomical plate)and a study group(locking plate),with 38 patients in each group.The surgical and prognostic outcomes,shoulder joint function,range of motion,and complications were compared between the two groups.Results There was no statistically significant difference between the two groups in terms of intraoperative blood loss and surgical time(P>0.05).However,the study group had shorter hospital stays and fracture healing times compared to the control group,showing statistical significance(P<0.05).Before treatment,there were no statistically significant differences in the Constant-Murley shoulder joint function scores(CMS)across the dimensions between the two groups(P>0.05).After treatment,the function scores for pain,activities of daily living,range of motion,and deltoid muscle strength in the study group were higher than those in the control group,with statistically significant differences(P<0.05).Before treatment,there was no significant difference in shoulder joint range of motion between the two groups(P>0.05).After treatment,the angles of flexion,abduction,internal rotation,external rotation,and elevation in the study group were higher than those in the control group,showing statistical significance(P<0.05).The incidence of complications in the study group was lower than that in the control group,with statistically significant differences(P<0.05).Conclusion Compared with traditional anatomical plates,the use of locking plates for internal fixation allows for quicker recovery,more significant improvements in shoulder joint function,greater shoulder joint mobility,and a reduced incidence of complications.
