IgA、IgG水平与呼吸道合胞病毒感染毛细支气管炎患儿的相关性分析

摘  要:目的探究免疫球蛋白A(IgA)、免疫球蛋白G(IgG)水平与呼吸道合胞病毒(RSV)感染毛细支气管炎患儿的相关性。方法选取2017年5月至2023年7月在本院接受治疗的80例RSV毛细支气管炎患儿作为研究组,另选取同期在本院进行健康体检的40例健康婴幼儿作为健康组,检测比较两组血清IgA、IgG水平。研究组患儿入组后均接受临床规范化治疗,根据治疗10 d的临床效果将其分为疗效优良组和疗效不良组,并收集、比较患儿人口学资料、病理资料,采用Logistic回归分析法明确血清IgA、IgG与RSV毛细支气管炎患儿疗效的关系。结果相较健康组,研究组患儿血清IgA、IgG水平均偏低,差异均有统计学意义(P<0.05);经Logistic回归分析结果显示,低IgA、IgG水平、出生时体重、特应性家族史、合并先天性心脏病史、并发呼吸衰竭、并发缺氧性酸中毒、并发心力衰竭是影响RSV毛细支气管炎临床疗效的影响因素,差异均有统计学意义(P<0.05)。结论RSV毛细支气管炎患儿血清IgA、IgG水平均表达异常,与临床治疗效果有密切关系,在临床治疗期间动态监测,可为掌握患儿病情提供参考。Objective To explore the correlation between the levels of immune immunoglobulin A(IgA)and immunoglobulin G(IgG)and respiratory syncytial virus(RSV)infection in children with bronchiolitis.Methods A total of 80 children with RSV bronchiolitis treated at our hospital from May 2017 to July 2023 were selected as the study group.Additionally,40 healthy infants and young children who underwent health examinations at our hospital during the same period were selected as the healthy control group.The serum IgA and IgG levels were measured and compared between the two groups.All children in the study group received standardized clinical treatment.Based on clinical efficacy after 10 days of treatment,the children were classified into an effective group and an ineffective group.Physiological and pathological data were collected and compared,and logistic regression analysis was employed to clarify the relationship between serum IgA,IgG levels,and clinical efficacy in children with RSV bronchiolitis.Results Compared to the healthy control group,the serum levels of IgA and IgG in the study group were significantly lower,with statistically significant differences(P<0.05).Logistic regression analysis revealed that low levels of IgA and IgG,birth weight<2.5 kg or>4 kg,a family history of atopy,a history of congenital heart disease,and complications were identified as risk factors affecting the clinical efficacy of RSV bronchiolitis,with statistically significant differences(P<0.05).Conclusion Children with RSV bronchiolitis exhibit abnormal serum levels of IgA and IgG,which are closely related to clinical treatment outcomes.Dynamic monitoring during clinical treatment can provide references for understanding the condition of the children.
