摘 要:目的探究多层螺旋计算机断层扫描(MSCT)后处理技术在眼眶外伤性骨折临床诊断中的应用效果。方法选取2023年1月至2024年12月洛阳市第三人民医院收治的76例眼眶外伤性骨折患者为研究对象,各患者先实施常规CT检查,后进行MSCT后处理技术检查,统计所有患者CT检查征象,比较常规CT与MSCT后处理技术的诊断结果(骨折部位、骨折类型),并以手术证实结果为金标准,分析两种方法的诊断准确率。结果眼眶外伤性骨折患者在CT检查下的直接征象为眶壁骨质连续性中断,检出65例(85.53%);纳入研究的76例患者中主要表现为以下几种间接征象:眼睑、鼻根、颜面部软组织肿胀56例(73.68%),眼睑、鼻根、颜面部皮下积气53例(69.74%),内直肌、下直肌增粗或移位42例(55.26%),筛窦小房变形41例(53.95%),筛窦积血35例(46.05%),上颌窦积血32例(42.11%),眶内脂肪疝30例(39.47%),眶内积气34例(44.74%),眼睑异物16例(21.05%),球后脂肪间隙模糊27例(35.53%)。常规CT与MSCT后处理技术对不同部位与不同类型眼眶骨折的检出率比较,差异无统计学意义(P>0.05);但MSCT后处理对各部位与各类型的总检出率高于常规CT,差异有统计学意义(P<0.05)。MSCT后处理技术对骨折部位、骨折类型的诊断准确率均高于常规CT检查,差异有统计学意义(P<0.05)。结论MSCT后处理技术对不同部位与不同类型眼眶外伤性骨折均具有良好的诊断效果,可显著提升诊断结果的准确性,为临床诊断提供参考。Objective To explore the application effectiveness of multislice computed tomography(MSCT)post-processing techniques in the clinical diagnosis of traumatic orbital fractures.Methods A total of 76 patients with traumatic orbital fractures admitted to The Third People's Hospital of Luoyang from January 2023 to December 2024 were selected for the study.Each patient first underwent conventional CT examination,followed by MSCT post-processing technique examination.The CT findings from all patients were recorded,and the diagnostic results(fracture location and type)from conventional CT were compared to those from MSCT post-processing,using surgical results as the gold standard to analyze the diagnostic accuracy of both methods.Results The direct findings of traumatic orbital fractures on CT included interruption of bony continuity of the orbital wall,detected in 65 cases(85.53%).The main indirect findings among the 76 patients included:swelling of the eyelid,nasal root,and facial soft tissues in 56 cases(73.68%);subcutaneous emphysema in the eyelid,nasal root,and facial area in 53 cases(69.74%);thickening or displacement of the medial rectus and inferior rectus muscles in 42 cases(55.26%);deformity of the ethmoid sinus cells in 41 cases(53.95%);hematoma in the ethmoid sinus in 35 cases(46.05%);hematoma in the maxillary sinus in 32 cases(42.11%);intraorbital fat herniation in 30 cases(39.47%);intraorbital air in 34 cases(44.74%);foreign bodies in the eyelid in 16 cases(21.05%);and blurred fat spaces behind the globe in 27 cases(35.53%).There was no statistically significant difference in the detection rates of different locations and types of orbital fractures between conventional CT and MSCT post-processing(P>0.05);however,the overall detection rate for various locations and types was significantly higher in MSCT post-processing compared to conventional CT(P<0.05).The diagnostic accuracy of fracture location and type in MSCT post-processing was significantly higher than that of conventional CT examination(P<0.05).Concl
