“去菀陈莝”理论指导针药并举治疗慢性湿疹

摘　　要：慢性湿疹是一种常见的炎症性、瘙痒性疾病,具有致病因素广泛、病程迁延难愈、症状反复发作的特点。本病以本虚标实为病机要点,多因脏腑虚损,气血津液运行不畅,导致气、湿、瘀血壅遏肌肤而发病,总结其核心病机为“气机不畅、湿浊阻滞、瘀血阻络”。“去菀陈莝”是祛除邪气、恢复经络气血调畅的治疗原则之一,文章结合慢性湿疹的病因病机,阐释该理论在遣方用药、针灸及刺络放血法中的临床应用,提出从气、湿、瘀论治,总结“调畅气机升降、益气健脾祛湿、清热凉血化瘀”的核心治法,并随证加减,以期为该疾病提供诊疗经验。Chronic eczema is a common inflammatory and pruritic skin disorder characterized by a variety of pathogenic factors,a prolonged and refractory course,and recurrent symptoms.The disease mechanism is primarily rooted in"deficiency in origin and excess in manifestation",often caused by visceral deficiency and impaired circulation of Qi,blood and body fluids,leading to accumulation of Qi,dampness and blood stasis in the skin.The core pathogenesis is summarized as"Qi stagnation,dampness obstruction and blood stasis blocking collaterals"."Eliminating Stagnation and Dispersing Accumulation"is one of the therapeutic principles aimed at eliminating pathogenic factors and restoring the smooth flow of Qi and blood in the meridians.Based on the etiology and pathogenesis of chronic eczema,this article explains the clinical application of this theory in herbal prescriptions,acupuncture and bloodletting therapy.It suggests treating the disease from the perspective of Qi,dampness and blood stasis,summarizing the core treatment methods as"regulating the ascending and descending of Qi,invigorating Qi and strengthening the spleen to eliminate dampness,and clearing heat and cooling blood to eliminate stasis".The treatment is adapted to the symptoms and aims to provide diagnostic and therapeutic insights for this disease.
