吴远华教授运用苗医“以通治痛”理论论治神经病理性疼痛临证经验

摘　　要：神经病理性疼痛被描述为由体感系统病变或疾病引起的疼痛,在苗医中可归属于七经中的“半边经(衣批嗯)”,翻类中的“喜鹊翻(架假翻)”的范畴,是苗医临床常见疾病。由于神经性疼痛的各种表现、原因和症状,这种疾病的治疗对临床医生来说都是极具挑战性的。吴远华教授结合自己多年临床经验,认为神经病理性疼痛的主要病机是各种毒邪侵犯人体筋脉,血脉瘀滞,筋脉绞结,筋气逆乱,不通则痛。在治疗方面,基于“以通治痛”理论,形成以祛毒散邪,通筋止痛为治法,并创新性地采用苗医“四大血”为领头药加减论治该病,经过多年的临床实践,取得了理想的临床效果。Neuropathic pain,described as pain caused by lesions or diseases of the somatosensory system,can be categorised in Hmong medicine as“half-border meridian(衣批嗯)”in the seven meridians and“magpie s turn(架假翻)”in the turnover category,and it is a common disease in Hmong medicine.Due to the various manifestations,causes and symptoms of neuropathic pain,the treatment of this disease is extremely challenging for clinicians.Combining his many years of clinical experience,Professor Wu Yuanhua believes that the main mechanism of neuropathic pain is that all kinds of poisonous evils invade the body s tendons and veins,resulting in stagnation of blood,strangulation of the tendons and veins,reversal of the tendon qi,and pain when there is no communication.In terms of treatment,based on the theory of“treating pain with communication”,he has formed the treatment method of eliminating poison and dispersing evils and relieving pain through the tendons,and has innovatively adopted the“Four Great Bloods”of Miao medicine as the leading medicine to add and subtract to treat the disease,which has achieved ideal results after many years of clinical practice.
