摘  要:胫骨近端感染性骨缺损因感染严重、骨缺损量大需要行多次手术治疗。随着手术次数的增加,患者会出现相关的骨及软组织并发症。目前临床多采用Papineau技术、Masquelet技术、血管化腓骨移植以及骨搬运技术治疗胫骨近端感染性骨缺损。此外,利用生物材料(如硫酸钙)作为抗生素的载体、3D打印生物材料、多孔氧化铝载抗生素陶瓷的应用也日益广泛。血糖控制、吸烟及饮酒情况、营养状况以及相关的内分泌疾病等因素均与疗效和并发症密切相关。因此,全面了解胫骨近端感染性骨缺损的诊断及治疗进展,可以为疾病的临床治疗及研究提供新思路。The proximal tibial infected bone defect requires multiple surgical treatment due to severe infection and large bone defect.With the increase of the number of operations,the patients will have related bone and soft tissue complications.At present,Papineau technique,Masquelet technique,vascularized fibular grafting and bone transport technique are commonly used in the clinical treatment of infected bone defects in proximal tibia.In addition,the use of biomaterials(such as calcium sulfate)as carriers of antibiotics,3D printed biomaterials,and porous alumina antibiotic-loaded ceramics are also increasingly used.Factors such as glycemic control,smoking and drinking,nutritional status,and related endocrine diseases are closely related to the efficacy and complications.Therefore,a comprehensive understanding of the diagnosis and treatment progress of infectious bone defects in the proximal tibia can provide new ideas for the clinical treatment and research of the disease.
